- no non-essential access to site or onsite employee parking
- getting PSI to have a sweeper on site or regular sweeping to better remove sediment that has built up in the gutters.
- will have two full length construction entrances and block off access to site where there is no entrance.

- request/reminder to notiify ESC divsion when dewatering for the stormwater ponds begins.

Meeting with new project manager.

Bentonite clay has been put inside the eroded hole on the slope to fill it up. The clay will expand and harden.

Other depressions will be filled with dirt to make a solid surface and then slope interupting wattles will be placed above and below the area.

Request to remove the above ground silt fence behind the home's fence. It is no longer needed and started to fall apart.

No other issues at site. Brett with Roth will notify ESC division when work is complete.

- No significant trackout from site access points at 1st & State and 2nd & Bannock. Runoff from disturbed areas flows into sites tunnel excavation area (minimizes potential offsite discharges)
- site is using Ecopan service for concrete washout
- trash is well managed, no loose trash laying around site
- site has sweeping unit working at access points. Active haul off occuring at Jefferson


Corrections requests:
- install gravel bag gutter check dam bags on Jefferson where CUP excavation & haul off is occuring to contain material in gutters close to site for easy manual clean-up
- check with St. Luke's facilty management regarding putting spill absorbent tubes or secondary containment around the trash compactors (due to leaks and oil drips)
- check that SWPPP has completed operator certfication paperwork complete.
- Inspection reports should be placed in SWPPP binder no longer than after 14 days of the date of inspection
- request during site training or employee meetings that offsite smokers do not throw cigarette butts on the ground but into a trash container or ash tray

- Install stabilized access to the Ecopan’s located on 1st street on east side of garage structure, scoop/contain mud along the curb
