Need to verify inlet protection to DI at gratz and auto drive. Contractor removed inlet protection due to plugging up the street with water and did not reinstall inlet protection. Contractor is to install new inlet protection.

No corrections made, muddy water flowing to inlet on Auto Drive.

No workers on site, will follow up tomorrow.

Need to stop sediment closer to site with sandbag check dams. Need to install construciton entrance or prohibit worker parking in the mud.

Discussion with homeowner RP regarding project and ESC permit conditions. Project to occur within confines of vegetated backyard that receives and treats potential stormwater runoff from area of disturbance. RP familiar with and aware of clean up requirements of any inidental trackout.

Significant excavation to begin the following weeks, project approved to proceed as planned, no site prep BMP inspection required.

General inspection of site conditions shall occer within 2-3 week.

Complaint called in to Public Works Hillside regarding polluted runoff from site.

Runoff was no longer flowing at time of inspection but there is evidence of a significant muddy flow and collected mud at 5th and Ada in particular:

Reason for BMP failure is that no new BMPs were installed to replace the sediment retention pond and wattles that were installed prior to the sewer work being completed. Runoff moved down the compacted road base picking up sediment and flowing onto Sherman street untreated.

Correction notice issued:
- clean up streets and intersection from site to 7th & Ada MS4 DI.
- install BMPs at bottom of site to control sheet flow runoff from higher areas.
- street sweeper shall be required.

Note: site has SWO on it due to grading re-submittal request. ESC plan currently under review. No workers or activity at site.

Syman Co. to respond to clean up issues this afternoon.

Driveway is now paved. Home near completion but not yet landscaped. No trackout, no turbid discharge or evidence thereof. Storm drain in front of home has sediment filter bag installed.

Stormwater improvements along HV Road handled rain storm well. No trackout, no subdivision work occuring, just Tahoe Homes working on SFH foundations. Permitted seperately. Lined washout being utilized.

Streets were cleaned after retaining wall operation and haul off last week. Storm drains have inlet bags, should be checked for maintenance this week.

No active trackout; wind blown trash and materials at south side of Warm Springs are picked up weekly, only a few items at time of inspection. NO violations observed, no hillside erosion issues.
