***See Attached ACHD Letter****(VERIZON WIRELESS CELL TOWER AND EQUIPMENT STRUCTURE) - To install a 67 foot tall steel monopole telecommunications tower and a 275 square foot, one story, pre-manufactured equipment shelter for electrical equipment and generator. The equipment shelter will be placed on a concrete slab foundation. The pre-manufactured shelter was reviewed and approved by the state of Idaho Division of Building Safety, PA#: MOD1403-00026. The monopole is a stealth tree design. The monopole and equipment shelter will be placed in a 1,355 square foot lease area, enclosed by a security fence. An existing residential structure was demolished and removed under permit BLD14-03555. All construction per the approved plans. JCM
