ALPHONSUS REGIONAL MEDICAL CENTER – EP LAB – MOB #6 – 3RD STORY) (DRH20-00109) (4-16-20 PLAN MODIFICATION TO ROUTE SUPPLY AND RETURN DUCTWORK AND REFRIGERANT PIPING FROM THE 3RD STORY ON THE EXTERIOR OF THE BUILDING TO THE ROOF TOP HVAC UNIT. THE DUCTS WILL BE SCREENED BY A DUCT CHASE THAT MATCHES THE EXISTING BUILDING FINISHES. WORK IS TO INCLUDE REMOVING THE EXISTING EIFS AT THE LOCATION OF THE NEW DUCT CHASE, CONSTRUCTING A STEEL STUD WITH GYPSUM SHEATHING CHASE TO ENCLOSE THE NEW DUCTS (SIDES AND BOTTOM) SUPPORTED BY HORIZONTAL STEEL “W” BEAM OUTRIGGERS WITH PERPENDICULAR STEEL “C” CHANNEL BEAMS, INSTALLING NEW DRAINABLE EIFS FOR THE EXTERIOR FINISH OF THE DUCT CHASE, INSTALLING A NEW METAL ROOF COVERING OVER THE ROOF DECK OF THE DUCT CHASE, CONSTRUCTING A NEW SCREEN WALL ON THE ROOF, AND INSTALLING NEW INSULATED DUCTS. SPECIAL INSPECTION FOR ANCHORS POST-INSTALLED IN HARDENED MASONRY AND FOR WELDING.) The fire sprinkler system is used to allow an Ambulatory Care Facility within this building. To remodel a 2,176 square foot area located on the 3rd story of an existing medical office building. The existing Ambulatory Care Facility will be expanded to include a new 1,262 square foot EP lab that is located within the remodel area. After the expansion, the Ambulatory Care facility will have a total floor area of about 5,529 square feet and will be separated from the rest of the story by a minimum 1-hr fire partition/smoke barrier. This permit is intended to establish a Certificate of Occupancy for the 1,262 square foot EP lab only within the expanded Ambulatory Care Facility. Work is to include interior nonbearing walls with adhered lead sheet gypsum board, a staff toilet room accessible to the disabled, a staff changing room accessible to the disabled, a staff dressing/locker room accessible to the disabled, 1-hr fire partitions, 1-hr smoke barrier walls and floors, a lead shielded sliding door, side-hinged lead shielded swinging doors, side-hinged swinging doors, interior leaded windows, a 2-hr mechanical shaft from the 3rd story through the 4th story, a new roof top HVAC unit, a suspended Unistrut grid for supporting overhead equipment, suspended acoustical ceilings, a suspended gypsum board ceiling, medical equipment, new medical gas piping in the pedestrian walkway from the main hospital, millwork, and finishes. Special Inspection is required for welding, bolting, and for post-installed anchors in hardened concrete. The work is required to comply with the approved drawings, with the structural design, with the plan review report, and with all applicable codes and ordinances. The fire sprinkler system is required to comply with NFPA 13 and the fire alarm system is required to comply with NFPA 72 as regulated by the Fire Department. Separate fire sprinkler permits and fire alarm permits shall be obtained prior to beginning any modification or installation work on such systems. **CJW**
