1. Bed the water service 6" below the pipe with clean soil and 12" above. Remove rocks from below the pipe.
2. The water service needs to be 42" below grade. Check depth at the service connection.
3. Sewer connection is defective at the cleanout. The pipe must fit straight into the joint. It's leaking.
4. Properly bed the sewer the entire length with clean soil.
