110.3- Power to chime transformer missed. 250.24(A)(5)- Grounded conductor shall not be connected to ground on the loadside of service disconnecting means. Remove grounding electrode conductor from sub panel.
314.17(A)- Open KO in entry 3/0 box. Shall close KO with approved means.
314.25- All boxes shall have covers. Recessed can in powder bath missing junction box cover.
