To construct 1 office and a conference room within an existing 12,213 square foot medical office building. Two existing restrooms are also being altered to be accessible to the disabled. Work is to include interior nonbearing walls, interior doors, interior windows, and suspended ceilings all per the approved drawings and plan review conditions on file. Sprinkler heads are overspaced on the Radiant Research side of the building in the sleep study area, outside the sleep study area on the north side and one pendant in the open office is too close to a column. Provide additional emergency lights as marked on the plans and repair the two emergency lights on the Selah side. Provide emergency lighting in the main lobby.

I spoke to the building owner Scott and notified him that I had left a notice that the affected areas as described in the open building permits and fire permits would be shutdown/closed for use if all permits were not finaled by 11/11/05. The notice was left with Diane at the front desk. I spoke to Scott over the phone. He was going to contact Odie with Steed and Jim Danes with DeBest.

Resubmit building plans showing as-built wall layout for the entire facility. Include correct room descriptions and locations of all rated walls, doors and glass. Room not shown on the plans in work area 157 has six bunk beds. Per on of the office personell this is used for overnight studies of insomnia. Provide description of this use and all other uses to the plan reviewer. Door hardware is not of approved type on some of the doors in the remodeled area and exit signs are not operating correctly. Atleast one sprinkler head is overspaced in the comference room. Pendant head in corridor outside of office 103 is too close to the wall (2") and a pendant head in storage room 134/135 exceeds 130 sq.ft. max. spacing.
