(DR JOHN BRUCE - EXETER CENTER BLDG C) 10/14/08 - PLAN MODIFICATION: TO EXCAVATE AND ENCLOSE A 290 SQ. FT. PORTION OF THE CRAWL SPACE FOR USE AS MECHANICAL SERVICE AREA. THIS NEW DEEPER CRAWL SPACE WILL BE ACCESSED WILL BE BY A 30" X 48" TRAP DOOR LOCATED UNDER IN THE FLOOR UNDER THE EXISTING STAIRS TO SECOND LEVEL. THE EXISTING FURNACES AND AIR COMPRESSOR WILL BE RELOCATED INTO THIS CRAWL SPACE AREA TO SERVE THE MAIN LEVEL AND SECOND LEVEL DENTAL OFFICE. INSULATION OF FLOOR BETWEEN CONDITIONED AND UNCONDITIONED MAIN FLOOR IS REQUIRED. THE REMAINED OF THE ORIGINAL PERMIT IS NO LONGER APPLICABLE IN RELATION TO A CRAWL SPACE AREA USED ONLY FOR INSTALLATION OF MECHANICAL EQUIPMENT. THERE IS NO OCCUPANCY OR STORAGE USE ALLOWED UNDER THIS PERMIT PLAN MODIFICATION. STRUCTURAL SUPPORT MODIFICATION AS PER REVIEWED MODIFIED PLANS. A 'LETTER OF COMPLIANCE IS REQUIRED FOR FINAL APPROVAL OF THIS SCOPE OF WORK PROJECT. To construct a 290 sq. ft. basement under existing 3,467 sq.ft. [3,012 s.f. main & 455 s.f. upper] two story building occupied by dental office (BLD03-01356) per approved plans. Basement (changed with modification to crawl space) area is to be used for mechanical EQUIPMENT INSTALLATION (NO storage OR occupancy USE ALLOWED BASED ON PLAN MODIFICATION THAT REMOVED REQUIRED COMPLIANT STAIRWAY ACCESS). Work includes temporary structural support of existing floor system; excavation of new (NO basement - changed by plan modification submittal) CRAWL SPACE area; construction of new concrete footing & foundation; concrete floor; structural frame walls; floor opening FOR CRAWL SPACE ACCESS TRAP DOOR (and NO stairway OR to basement per plan modification); relocation of mechanical equipment to (basement) CRAWL SPACE; interior finishes, (NO handrails, doors and hardware - PLAN MOD); envelope compliance per energy code between conditioned and unconditioned space; (NOT REQUIRED AS NO STORAGE OR OCCUPANCY FOR MECHANICAL CRAWL SPACE - one hour fire resistive separation on the basement side ceiling, steel beams and wood frame support perimeter walls between storage and upper level dental office areas); and electrical & mechanical system upgrades.
