(Evergreen Plaza North End Concrete Footings / Sidewalk) Evergreen Plaza, LLC, proceeds at their own risk, with no guarantee that future permits for proposed canopy or openings in existing 2-hour fire wall will be issued. In the event that: (1) structural corrections are required of the concrete footings / piers / anchor bolts being installed under this permit, based on future submitted engineering design for proposed canopy and openings in the existing 2-hour fire wall, require demolition and replacement; or (2) the property exchange does not happen and work done under this permit requires removal; Evergreen Plaza, LLC, will be responsible for removing any structure that have been constructed or installed. Scope of work includes preparation and pouring of seven (7) reinforced concrete footing / piers / anchor bolts with concrete sidewalk from existing north wall, over the footings to a point 15 feet north of the existing wall. The sidewalk will extend the entire length of the existing north wall. A protective wood cap will be placed over the bolts and painted yellow to warn of tripping hazard. There will be no construction beyond this point under this permit. Letter provided by Kevin Booe, Director of Library; William Matter, representing David Thompson, owner of Evergreen Plaza, agreeing to this scope of work while ROS09-00004 that will record the adjusted property lines is in progress of being finalized; and conditions & limitations letter signed by Jason Blais, Building Official, are attached to and are a part of this permit. Site preparation of bearing soil, forming, reinforcement, concrete and anchor bolt placement are being inspected and tested for compliance with submitted plans based on Section 1704, Special Inspection.
