reason for fail is to keep permit open until a permit is pulled on work that was done at this location by persons without an electrical contractors license.
once permit is pulled and inspection scheduled for said work this permit will be able to be finaled. all work done on this permit is done and does not reflect negatively against contractor.

20 Light fixtures and most receptacels were installed by the contractor,NOT by the electrician, these items were done with out our knowledge Inspection requested via web by Matthew White...
1. electrical permit was pulled for service change only...work was performed beyond scope of permit and fees need to be updated for this work
2. electrical work at this address was done without an electrical permit being pulled. Work was done by a unlicensed person, electrician, electrical contractor. An electrical permit shall be pulled to cover other work being performed outside electrical permit ELE16-03139
