No earth disturbing work at this time. Project scheduled to be completed by end of December. No inspection conducted at this time.

Terraces construction is still in progress with interior work on the east building as well as lanscaping. Minor amount of trackout from laydown yard due to construction entrace and Lysted street has recently been swept.

DI bags installed until all areas stabilized; no fugitive litter or dust from site. No corrections at this time.

The west end of the Terraces is landscaped, paved and construction is complete. Residents are already living in the condo units and some of the hospital building.

Construction is remaing along the east end of site. All construction traffic is using the east entrance off Lysted. No significant trackout from site or laydown yard.

All DI bags are installed around the perimeter, so will need to make sure Petra removes them when complete with disturbed areas.

Terraces is in good condition. No fugitive dust from vehicle traffic or construction. No discharges or trackout around perimeter. Materials are contained. No complaints received yet this summer from surrounding residents. Approved.

Terraces site perimeter recently swept by ACHD trucks. Warm Springs, Council Springs, and Lysted are in good condition.

East site exit is being preped for paving. Recieving inlets have DI filter bags installed. Silt fence along southern project boundary has silt fence installed and should remain so until bank behind condos is stabilized and irrigation isn't causing rills.

No dust issues observed or complaints received. No spills or trackout

No significant trackout from either entrance to Terraces. Surrounding streets and gutters have been recently swept. No corrections at this time. WIP>.

Requested that Lysted Street south of construction entrance receive special sweeping attention today during the on-site sweeper rounds. It appears it has not been swept since last week, or new dirt is tracked out each day.

Petra supervisor JC stated that this afernoon it will be taken care off. Rain is forecasted for tomorrow.

Rest of site is in good condtion with no major trackout or loose sediment areas.

Terraces silt fence perimeter and DI protection BMPs are in good condition.

There is some trackout on Council Springs and Lysted, but inspection has been performed prior to daily cleanup. No significant accumulations, mud clods or discharges. Will check road conditions through out week but appears Warm Springs has been recently cleaned.

No corrections at this time.

Corrections not yet complete. Weather conditions making sweeping difficult.

Street shoulders are still very dirty and do not appear thoroughly swept. Construction entrances have not yet be repaired/turned over. Some sweeping did occur however as stated. New mud clod trackout from laydown yard (due to entrance conditions) through neighborhood to Council Springs entrance also needs cleaning.

Will check back. Extremely muddy and difficult conditions. Storm drains do filter bag protection but no sheet flow runoff occuring currently.

Terraces took good precautions to prevent trackout and sediment runoff today with the rain. No muddy discharges to storm drains or trackout observed.
Precautions taken:

1. East gate was locked to prevent trackout to Lysted.
2. Limited vehicle traffic to hard surfaces only
3. Employee parking areas have been resurfaced with rock and gravel to prevent trackout and creation of large mud puddles.

No corrections needed

Parking lots in staging area and west side parking lot have been cleaned up and have rock base installed to keep mud down. Much improved. Silt fence and DI protection in good condition.

Significant amount of loose sediment on Council Springs and surrounding area so still getting a lot of trackout of sediment from the the west entrance to Terraces.

Sweeper was onsite, but site still needs to continue to make changes to prevent trackout.

Site meeting requested to discuss CN issued 12/10. Petra, Idaho Sand & Gravel, Syman Co, and City of Boise ESC division will be in attendance

Meeting summary:

Correctiive actions already completed/in progress:
1. Sweeper with vacuum swept the perimeter roadways and removed loose sediment that had accumulated on Warm Springs.
2. Crew has started cleanup of dirt/mud in gutters.

Meeting summary & conclusions:
1. Material delivery will be restricted to mornings before 10 AM. The idea is that during winter, the morning may still have frozen ground. Also, after 10 AM the east construction access will be closed so that any clean up of trackout need can be completed only once that day without fear that more trackout will be caused by next delivery.
2. Designate a clean/mud free forklift or material hauler to ferry materials from laydown yard to main site. Muddy forklifts should not leave the main site.
3. Syman Company was at meeting and will be maintaining all drop inlet bags and the section(s) of damaged silt fence. Syman Co. is handling SWPPP inspections every 2 weeks and after rain events. There is a process in place for Petra to review and respond to inspection findings/deficiencies. Syman may consider switching to weekly inspections.
4. Employee parking is to be moved to the laydown yard where there is a good gravel & rock base. Until this time, the sweeper will be cleaning the parking lot exit.
5. Pit run access roads have been installed on site by Idaho Sand & Gravel. Worker training, signage, and installing fencing or caution tape to keep vehicles on these roads and out of the mud will cut down on dirt on hard surfaces.

Site conditions have improved since yesterday. All representative at meeting were in agreement on the plan for corrective actions.

Will continue to monitor and check progress.

Significant trackout at both construction access points and onto surrounding roads

Correction:
Main access road through site needs good cleaning and need to designate staging areas or parking areas to reduce amount of dirt on this road.

Some issues will be taken care of by the regular sweeping that is occuring, but will need to take advantage of the drier conditions this week to prevent future issues.

Will monitor next few days to see if conditions improve.

Perimeter BMPs and inlet protection are installed and satisfactory. The interior road is paved but very dirty, with rain the site would discharge muddy stormwater. Site is said to sweep two times daily, but does not appear to be occuring. Unstabilized dirt at east entrance on Lysted is causing trackout and sediment accumulating in gutter.

Need to improve housekeeping and contain the sediment on Lysted street.

Terraces of Boise surrounding streets and gutters are free of trackout from construction entrances and in the gutters. DI bags installed on all inlets with remaining capacity. Lysted St. has been temporarily patched to stabilze area in front of east side access point. No dust issues at time of inspection. No corrections required at this time.

Perimeter streets around Terraces project have been swept as requested last week. Syman Company also maintained silt sacks in storm drains. No new trackout or ESC issues observed. Corrections complete

Correction Notice:

1. Sweep Council Springs at west construction entrance as well as the first block Warm Springs to the west of Council Springs.
2. Sweep Lysted and install gravel bag where the pavement is not patched to collect loose sediment and rocks.

VM left to JC of Petra. Will follow up. Requested to complete by end of day 9/4.

Some track out occuring on Lysted. Called Preston to disclose to him that a new construction entrance needs to be installed and Lysted needs to be swept around the round-a-bout

Terraces project is in good condition. Light suppressed dust and water truck is on site, but traffic has also decrease. Silt fence perimeter has no breaches, both entrance has heavy duty rock, permit and SWPPP on-site. There is some utility work on Lysted that is not yet patched over and is a small source of loose sediment. Should be covered up soon.

Observed that the correction discussed on Friday 5/9 has been completed.

DI on Warm Springs and gutter has been swept with sand and dirt removed.

Site is currently in compliance.

Follow up inspection to see if action items from the 5/1 conference were completed.

Both entrances have new rock that is also extended. Very little sediment accumulation in gutters along Lysted. West entrance on Council Springs does not have any significant track out.

Site conditions are much improved. Need to check if DI on north side of Warm Springs between Council Springs and Eckert. Lots of sediment in gutters, unclear if it is from Terraces project trackout in preveious weeks - numerous projects are in the vicinity. Will follow up. No corrections at this time.

Conference organized at Terraces' job site Main Office to discuss trackout issues from site and the Correction Notice (CN) issued on 4/30/2014.

Summary: Petra organized a meeting at 8:30 AM on 5/1 to allow the ESC division to discuss track out issues with subcontractors on site. After the meeting we discussed plans to control trackout going forward and the corrections already made/in-progress.



Corrections made:

1. Round-a-bout on Lysted and entrance area on Council Springs have been swept and are in good condition.

2. Accumulated sediment in gutters has been scooped and removed manually.

3. Construction entrances to main site have been restored and lengthened, the staging yard entrance will be improved once more rock arrives

4. Subcontractors were made aware of the issue of trackout and how they can help reduce trackout.



Plans going forward:

1. The west entrance is only to be used for entry only in effort to prevent trackout onto Council Springs. The roundabout will be the main exit with the more serious trackout controls in place.
2. Street sweeping to occur 2 x daily.

3. A crew has been specifically designated to sweep and shovel the gutters where sediment accumulates.

4. Syman Co. will continue to make SWPPP inspections and email the stormwater team the corrections necessary (JC, Preston, Brett, John)
5. Boise ESC will always be sure to email entire stormwater team in addition to phone calls or conversations so everyone is aware of Correction Notices issued so they may be properly responded to.

With the corrective actions listed above and conference organized, the Terraces of Boise site is now currently in compliance.

Going forward the two ESC permits for Terraces will be:


ESC13-00755 : main site

ESC13-00793: staging/laydown yard
ESC13-00069 was issued prior to these permits and will be closed to simplify record keeping and communication. Note: This permit covered the acreage fees for the entire main site.

Significant trackout. No corrections visible after multiple notices given to Preston (ISG) and JC (Petra) during previous week.

Went into job trailer and spoke JC. He explained some of the issues the site has been having with trackout. The amount of rainfall last week was exceptional and the entrance were not able to be restored as requested, which is understandable.

Idaho Sand & Gravel is supposed to restore both entrances and sweeping is said to be done daily but hasn't been very effective. Perimeter BMPs are good, but trackout due to degraded entrances and weak sweeping is the main problem.

Entrances and sweeping are supposed to be done this afternoon. Will continue to monitor.

No observable improvement in track out at both entrances. Pictures were taken. This is document continued non-compliance in regards to construction entrances and SWPPP narrative submited for permit.

Follow up on the Terraces project. Roads are being swept 1-2 times per day according to foreman JC Murray. Trackout at roundabout is not bad, but need to sweep entrance at Council Springs today.

JC stated that today more pit run is being brought in to increase length of constructions entrances at both ends, if not through the entire site.

Need to continue sweeping. Will check silt fences next week.

Corrections required:

1. Sweep roundabout at main construction entrance and all the way to Warm Springs
2.. Sweep in front of western construction access
3. Restore construction entrances with rock
4. Provide alternate wheel wash area to prevent pooling at entrance.

Perimeter of Terraces looks good, but need to sweep roundabout to Warm Springs and replenish construction entrance and drain puddle that is inside site before construction entrance.

Verbal CN issued to Petra RP Preston. History of response to corrections has been postive so far.

The Terraces Perimeter BMPs in good condition. Construction entrance preventing trackout. Approved.
