STOP WORK ORDER ISSUED: Lack of BMP Maintenance, BMP's Inadequate for Slopes, Severe Turbid Discharges occuring throughout site. MS4 Severely Impacted with Heavy Sedimentation, Sidewalk, curb and Right-of-Way all impacted with Sediment. Took Photos and Sending them to Jenifer and Andy. Collin stated there will be no work occuring on this site for 2 weeks. I told him NO WORK shall take place, until violations are corrected, Reinspection approval Required prior to the Lift of Stop Work Order

Significant Amount of Sediment leaving various areas of the site into ACHD ROW and MS4. Met with Colin onsite, with one of his subs. He began installing gravel check dams. He is to install construction entrance, more Perimeter BMP's along sidewalks and clean out the Drop Inlet. Once things dry up, he shall sweep entire Bent Bow Road, curbs and sidewalks.

Meeting discussed corrections to be made at end of Bent Bow and perimeter. Lots that have been graded but do not have current construction activity permits will be accomplished by Wood River and Canyon Crest whom own some of the individual sites.

Contractors have been given several days to have BMPs delivered and installed. Sweeping and DI bag clean out will be addressed today.

Complaint called to Evan Jenkins. Evan and I inspected site. Sediment has washed into street and MS4 due to lack of BMPs installed at downslopes and perimeters.

Corrections:
1. Clean up sediment on sidewalks and streets
2. Replace DI bags
3. Install BMPs in affected areas
4. Need to stablize open lots with hydroseed or mulch
5. A conference will be scheduled.
