Report of no fugitive dust control

No dust control at time of arrival. Front end hoist driving from alley on Myrtle to Capitol creating dust. Lots of loose dirt in staging lanes and at perimeter. RP on permit no longer on site, no new contact provided.

Dustin with Tuffy did order water truck, push broom sweeping of the drive lane, and told equipment operators to drive slower.

Will check back Monday on conditions. Storm drain system on Capitol/Myrtle has inlet protection and will be retrofit next week.

Myrtle Street cleaned/swept as requested in previous inspection. Masonry crew still on site working, scheduled to be done this week. Area to be cleaned thoroughly once their staging area is removed.

Uncontained masonry cement mix station next to gutter. Gutter has dirt, mud, and cementitious materials. MS4 inlet does have filter inlet bag; concete washout bins are being utilized.

Onsite inspection and verbal correction request to RP Jim:
- clean up gutter, sweep staging lane within fence
- install checkdam sand bags to trap materials on the surface for easy maintenance.
- coir mat filter on top of inlet in additon to filter bag could add protection.
- increase sweepig of the area to 3-4 x a week

Jim agreed and started getting crew to make corrections and clean. Will re-inspect early next week

Site preveiously prepped and is approved for staging for footing work to begin in early January. Tuffy and Big D excavation on site.

Corrections:
- install 1 DI bag on Myrtle and 6th to receive runoff from Broad Street east of construction by Trader Joes. All of DIs have inlet protection.
- site is already pit run should not be too muddy, but construction entrance may require additional rock or rumble bars.
