Associated building permit: BLD09-01275 Work is to include interior nonbearing
walls, interior doors, interior windows, 2 restrooms accessible to the disabled, suspended acoustical
ceilings, millwork, and finishes. The work is required to comply with the approved drawings, with the
plan review report, and with all applicable codes and ordinances. The fire sprinkler system is required to
comply with NFPA 13 as regulated by the Fire Department. A separate permit is required for high-rack

Duct smoke detectors tested ok. Verification was provided for the factory installed duct smoke detectors. Need corrected map at the facp, sticker at the annunciator as discussed and nfpa 72 record of completion forms.

The hvac units apparently have not been connected for shutdown upon alarm activation. Supposedly will be wired/connected tomorrow and the inspection will be called back in. Re-inspection fee applied. A re-inspection fee was applied previously as well.

All items have been corrected. The hvac units had not been programmed yet so shut down could not be tested. The hvac installer needs to provide verification that the smoke duct detectors are factory installed or per System Sensor tech support the sample tubes will need to extend 2/3 of the way across the duct. As installed they do not.

All pull stations have been mounted too high and need to be relocated. The smoke detector above the panel is too high and needs to either be relocated or removed (it is not required in a fully sprinklered building). A notification device needs to be added to the hallway between the reception area and the kennels. An over-spaced notification device in the Salon needs to be relocated. There was no canned smoke available to test the duct detectors. None of the pull stations were tested at this inspection. All other notification device locations and functions were verified. Other signals tested and verified at this inspection were: the sprinkler tampers and flows, the loss of power for the main FACP as well as the remote power supplies, and the loss of phone lines.
