2- need to secure fireplace flue to structure 1st floor chase above fireplace
3- firecaulk missing 2nd floor at floor on fireplace flue
4- insulation shield needs 1" clearance and flue needs 1" clearance to wood in attic/ remove metal in attic around fireplace flue and replace with flat metal in place has piece from thimble and firecaulk all the way around pipe
5- missing seal on supply duct 2nd floor on both seams in chase
6- exposed insulation in crawl needs vapor barrier/taped at crawl entrance
