created this mis to track required cross connection control compliance. dropped off a correction notice at Perkins. corrections were originally addressed on a survey by United water and plumbing inspector from 7/14/09, associated with PLM09-01103.
*CORRECNTION NOTICE*
On 7/14/09 a cross connection survey was preformed at the above address with United Water. The following is a list of measures required to bring this building into compliance. Action must be taken with in 30 days. (1) watrer softener drain requires an air gap. (2) chemical station at the mop sink requires a seperate water supply. (3) R.P. required on the ice mechine upstream of the filter. (4) dish washer requires an R.P. (5) juicer requires a DCVA. (6) Hot Cocoa maker requires a DCVA (7) R.P. required on the water supply to the coffee maker, hot water dispensor and Ice tea maker. All the above is to be done by a licensed plumbing contractor, permitted and called in for inspection.
