Inspector to verify the construction and repair complies with Type II-1 hour constructionas non-combustible framing with fire code sheetrock cover; the duct penetration(s) of the ceiling have ceiling/fire dampers (existing per plans on file); that the door to the new waiting room area that is now part of the open corridor is a listed 20 minute smoke & draft stop assembly (per current code if not existing assembly). 5/26/00 - Have talked with Dennis Adams concerning this project and had Brian McMahon follow up with them on the fire department question concerning door lock downs in all areas when all areas are not classified as I-1.1 Occuapncy. Also Mr. McMahon wrote them up for working without a permit and converting an office into part of a reception room off of a corridor. Mr. Adams says they have architects on retainer working to analys the structure and the actual occupancy use(s) and the upgrades required to bring into compliance. Brian met with Bob Hanson of SARMC on the 16 of May and said the architects were scheduled to review the site on the 17th. Plans would be forthcoming.

Reviewed original plans for type of construction and occuapncy. Was a II-1-hour; I-1 occupancy and later an I-3 occupancy for dention of youth held in restraint do to physical, mental or emotional problems. Plans indicated the roof/ceiling assembly was a 1-hour system throughout and called for fire dampers at all ceiling penetrations. It was hard to determine if the area work was done in was a rated corridor but appeared to serve an area with an occupant load > 30. This wikl need to be verified by field inspection & architects analysis.

Questions on area work done in, the need for fire-resistive corridor & roof/ceiling assembly actual conditions. Hopefully this can be answered by field inspectors review on 1/5/00 with project engineer. I will still need a Certificate of Value and application for the permit.

Called Mr. Hanson to let him know the plans did not contain the entire value. An application or C.V. was not submitted. The location of the project could involve an original rated corridor and the requirment for upgrading the original office door to be a 20 minute assembly if not currently of such rating. There is also concern about the roof/ceiling assembly and ceiling fire dampers at duct penetrations. I siggested that Mr. hanson meet with Carl Dependahl to review the site and scope of work done and I would review findings with inspector. I called Carl to request he call Mr. hanson to set up time for inspection/review. The inspection can be tied to this permit as a preconference meeting.

Called Mike Gorringe, contractor for demolition. I ask him what he had done and why a permit was not taken out. He had been hired by the hospital and was told a permit was not necessary based on his bid of less than $500.00. I ask him if he had be hired to repair the flooring & paint the demoed area what his estimated costs would have been. He estimated another $120.00. This was only an estimate but would put the project value at >$500.00. I suggested that future projects should be verified with the city before work starts to see if permit is needed.

Call from Dennis Adams at SARMC to question requirement for permit for work less than $500.00 I told him it was our determination (Boise Building Dpet.) to review scope of projects and decide if permit was nedded. I ask for plans and application with C.V. for total costs of project, not just demo bid.

Received call from Bob Hanson, SARMC Project Engineer. He stated that a 5 foot hole was opened up in wall to create a walk through opening and that the cost of the demo was less than $500.00 and he didn't think a permit was required. The remodel created a larger waiting room by converting an existing Dr. Office into waiting area. I told Bob that the city ordinance did not specifically state that a permit was not required and if he added the total costs of the repair and finish to the demolision wotk it appeared the const would exceed $500.00. I ask him for p[lans showing what work had been done where. Question on if work done had invilved a fire wall? Bob did not know for sure but hadn"t thought so because the wall did not extend to the roof deck and the corridor was not tunneled. Bob to provide plans and application for review and possible permit.

Call from Brian McMahon, FD, concerning work done in field by Gorringe Bros. at 131 Allumbaugh, recently purchased by SARMC and called the Saint Alphonsus Behavioral Health Services Facility. I told Brian I had not been contacted concerning this project. Brian had told staff to contact the project engineer, Bob Hanson and provide plans & obtain a permit for the work done. Brian aske that I call him back if I do not here from ST.AL'S ASAP.
