(IDAHO ELK'S REHABILITATION HOSPITAL) To perform a remodel within the existing first floor Maintenance Administration & Shop Area, located in the SE Corner Area. This area is separated from the adjacent "I1.1" Hospital Treament area by an existing 2-hour Occupancy Separation wall. All 2-hour walls & rated opening / penetration protection SHALL be maintained or provided if not in place as part of this permit. The scope of this permit involves construction of interior non-combustible, fire-resistive, non-bearing, walls to create two storage rooms from one room, D1144 & D1144A, W/ new 60 minute door assembly to new storage room; new walls to create private office areas, house keeping room, maintenance shop rooms and access hall. Walls to extend from finish floor to new suspended ceiling systems above office areas & hall. No ceiling in the maintenance shop rooms. Uprade and or modify the electrical, mechanical, sprinkler and alarm systems as required to meet needs of occupants and to maintain all areas in compliance with latest adopted code of City of Boise. NOTE: This building is fully sprinklered. FIELD REVIEW, INSPECTION AND APPROVAL FOR LIFE SAFETY, SPRINKLER & ALARM SYSTEM COMPLIANCE, BY THE FIRE CODE FIELD INSPECTOR, IS REQUIRED AS A CONDITION OF THIS PERMIT. A "Letter of Completion / Compliance" is required as condition of final approval & occupancy / use of this remodeled area. PC#01-282/drs

Fee's still owed for PW BLD98-00062, $350.00. Hold placed on parcel by Linda Hartwell. When Hold removed or approval received to issue permit all will be per approved plans, notes on plans and attached plan review comments.
4/11/01 - Steve Turney, ZGA architect stated that he had notified the Elk's & Tim Wilcomb, Jordon-Wilcomb Const. concerning the fee needing to be paid. Tim Wilcomb will pay past due fee at the same time he picks up this permit. I reviewed this with Marsha Smylie so she would understand the situation. Linda Hartwell was at the dentist. Marsha has the plans for the new remodel under this permit and a copy of the pwk98-00062 fee due.

VISITED WITH LINDA HARTWELL CONCERNING THE PARCEL HOLD ON THIS PROJECT FROM 1998 PWK00-00062. LINDA GAVE ME A COPY OF THE FEE DUE WHICH I WILL FAX A COPY OF TO STEVE TURNEY & LET HIM KNOW WHO TO CALL REGARDING THE FEE'S. LINDA SAID THEY SHOULD BE PAID BEFORE OTHER PERMITS ARE ISSUED.

Call to Steve Turney to let him know that a parcel hold had been placed on the Elk's and that past due fee's from a 98 PW project were still outstanding and needed to be addressed before an additional permit can be issued. Steve was in a meeting so I left him a voie mail and faxed him a copy of the billing I had received from Linda Hartwell and gave him Brian Murphy's phone number at PW.
