(Trey McIntyre Project) To remodel the interior of a portion of an existing building for this tenant. The tenant consists of one large open room with a restroom corridor. Work to include: light demolition of some existing walls, construction of (2) interior non-bearing walls, install (3) new doors, modify (1) existing door, upgrade the lighting layout, build a sound barrier wall, modify the existing exit lighting, and install a new dance floor for the practice studio. Modification to the existing fire sprinklers and alarm systems require a seperate permit and are not part of this permit. Note: This building is fully Fire sprinklered , any modifications or alterations to the Fire Sprinkler system shall be per NFPA #13 as regulated by the Fire Department. All work per the approved plans, attached plan review report, and conditions of all reviewing agencies for this project. **SRZ**

On site meeting with Mark Senteno, Tim Wilcomb, Pat Gill, their fire alarm contractor to determine extent of fire alarm work required under the Foothills School permit FIR02-00256. That deferred upgrade agreement dated Nov. 20, 2002, signed by Dick Wilcomb, building owner and Dave Miller, Fire Marshal, committed the owner to providing additional manual pull stations within 2 weeks of the signed agreement and to provide horn/strobes throughout all areas of the building no later than 11/2/2003 and allowed immediate occupancy for Foothills School. It was determined at this 2/11/09 visit that the alarm work was not completed. Trey McIntyre has occupied the space and there is a new room in the main office which does not have sprinkler protection. Tim agreed to remove the ceiling of this room. He also agreed to have the fire alarm contractor install the required notification appliances throughout the Trey McIntyre space, including their office space, the public restrooms and hallway and the dance studio room. It was also agreed that the basement which is only used for owner's personal storage would only need audible alarm notification. An adjoining 6,000 sf vacant space within the same fire area, will also only need audible notification at this time due to being unoccupied. It was agreed the contractor will immediately submit an application for a fire alarm permit immediately, using the previous FIR02-00256 alarm plans as a starting point for the required alarm modification work. It was also noted the PFE's are out of date and need to be serviced.

Met with Angie Brosious to point out that no sprinkler or fire alarm permits have been applied for. She advised the architect. I followed up with Jeff Smith at E-squared Electric, who indicated that his engineering "layout" had been given to the electrical contractor to install on a "design-build" basis, using Rob Ahten of Protex as the designer. Jeff indicated he will follow up with the electrical contractor on Protex's status on the project.

Telcon from Jeff Smith, E-squared Engineering regarding fire alarm requirements. He is doing the fire alarm design work; plans on installing a/v devices within the TI space. He did not see any a/v's in this space or the other core areas he walked through, so it appears the upgrade ageement work has not yet been done.

Tim Wilcomb, building owner, ###-####, returned my phone message inquiring about the status of the fire alarm upgrade agreement, dated 11/20/2002. His voice message indicated audio visual devices were added in the BSU and UI spaces when they moved in, and he was of the understanding that all the upgrade work had been done at that time, under those permits.
