Received alternate request letter with floor plan asking for alternate means of providing water to the public by means of a bottle water cooler placed behind the reception counter. The receptionist will provide cups of water upon request. The Dr. deals with patients severly limited in ability to act and to understand and having the drinking fountain out in the waiting area will provide a possible source of injury and or water damage to the facility if patients play with water. The required HI-LO drinking fountain has been plumbed into the wall per original plans for future installation per alternate letter. A plan showing the proposed location of bottle water is attached to alternate letter. Jason Blais, Building Official approved the alternate and attached to this BLD10-01041. i FAXED A COPY OF THE APPROVED ALTERNATE REQUEST AND FLOOR PLAN BACK TO SHANNON at Dr. Wechslers office for their records. A copy is also attached to the original plans.
