2) Provide a completed Food Establishment License application and Food Establishment Plan Review and Approval form. The forms are available at http://www.cdhd.idaho.gov/EH/food/forms.htm
3) For the Public Works Department, provide a menu of the food that will be served
4) Provide revised Sheets M1.1 and P1.0 that match the revised architectural drawings. Make sure that you give the new sheets the exact name as the former sheets so they come in as a version. The revised sheets must be stamped and signed by a Mechanical Engineer.
After these items have been uploaded, the plan modification can be reconsiderd.
