Per condition # 3 (a), we have a copy of the Reclamation Plan, please submit approval letter from Idaho Department of Lands. (b) Provide copies of the complete Notices of Intent for coverage and EPA acknowledgements of receipt for the Multi-Sector general Permit and Construction Permits for stormwater from the EPA. There is no site plan. Upload site plan as submitted for CUP14-00051 and indicate everything that is proposed with this permit.

9. Planning & Zoning letter(s) of approval– i.e. Conditional Use Permit (CUP), Design, Review Permit (DRH) and/or other approval documents.
