1) The #518 application and checklist can only be used for restaurants when a new restaurant is replacing an old restaurant. The space for new Pizza Hut was not a restaurant before. Please resubmit using the #520 application and checklist and provide all of the required documents listed on the checklist.
2) Provide the approval form from ADA County Central District Health Department (CDHD).
3) Provide an Erosion and Sediment Control plan or a waiver letter as the scope of work requires saw cutting concrete greater than 40 linear ft..
4) Provide a "Key Plan" on the cover page of the plans.
