James, I have routed this project for review. As this is a medical office you will need to have documentation from ACHD in regards to impact fees. It can be an email from them. I need to know that there are no additional fees, that any additional fees have been paid or will be paid at issuance of the permit. Medical office has a higher trip rate than standard office on ACHD fee chart.
