1. PDF Application for Suite 100 with tenant name, square footage, occupant load, and value of work specific to tenant suite 100.
2. PDF Application for Suite 101 with tenant name, square footage, occupant load, and value of work specific to tenant suite 101.
Additionally, please resubmit Form #304 so Registered Design Professional name on is the architect signing drawings (Andrew Erstad).
