(ST. AL’S HOSPITAL – LINEAR ACCELERATOR VAULT – 1ST STORY – NORTHEAST AREA OF NORTH TOWER) The fire sprinkler system is used for high rise and for occupancy. Interior demolition including the removal of the existing vault door and the surrounding concrete, the removal of some wall furring, and the removal of the existing ceiling is also covered under this permit. To remodel an existing 877 square foot linear accelerator vault and the associated 162 square foot control room (1,039 square feet total) to accommodate an equipment upgrade. Work is to include a concrete slab to fill an existing shallow pit, interior nonbearing walls, interior doors, a new leaded wall, wall surface protection, wall finishes, resilient sheet flooring, carpet tile, suspended acoustical ceilings with lay-in ceiling panels and translucent ceiling panels, a structural steel header at the enlarged vault door, and a new vault door. Special Inspection is required for anchors post-installed in hardened concrete. The work is required to comply with the approved drawings, with the structural design, with the plan review report, and with all applicable codes and ordinances. The remodel area is located entirely within an existing care suite and smoke compartment. The fire sprinkler system is required to comply with NFPA 13 (including the installation of quick-response heads within the remodel areas) and the fire alarm system is required to comply with NFPA 72 as regulated by the Fire Department. Separate fire sprinkler permits and fire alarm permits shall be obtained prior to beginning any modification or installation work on such systems. **CJW**
