**TERRY REILLY HEALTH SERVICES** This building is not fire sprinkled. To enclose a roofed area (364 sq. ft.) of an existing health clinic and remodel portions of the interior. This permit is for Phase 1. Phase 2 is required to start within 24 months of Jan. 30, 1996. The total new floor area is 3,857 square feet on the main floor and 600 square feet in the basement. The use of the basement is limited exclusively for mechanical equipment. Storage is not approved because of noncomplying exits. Work is to include footings, foundations, columns, beams, floor system, windows, restrooms accessible to the disabled, and interior walls all per approved drawings, engineering, and plan check notes on file. Separate electrical, plumbing, and mechanical permits are required. A CERTIFICATE OF OCCUPANCY SHALL NOT BE ISSUED UNTIL AFTER PHASE 2 IS COMPLETED. PC-319-95 **CJW**
