To remodel the interior of five existing common use toilet/shower rooms. Based on Div. II-Appendix Chapter 11, 1994 UBC, one unisex toilet facility is required per Sec. 1111.2.5. Work to include repair of walls and flooring, damaged by water & use and to install new fixture's to be usable by disabled. Work does not include any alteration of the bathroom entry doors per contractor. Any alteration to doors or supporting walls shall require full compliance of all toilet facilities for accessibility & usability per Chapter 11, 1994 UBC. All per approved plans and notes. SPOKE WITH GILBERT POND WHO STATED THAT THE ORIGINAL GENERAL CONTRACTOR (J & G) WAS FIRED FOUR MONTHS AGO AND POND HIRED TO COMPLETE THE JOB. DAN STUART INDICATED THAT HE WOULD NOT REQUIRE ADDITIONAL PLANS AS LONG AS THE NEW CONTRACTOR HAD ACCESS TO THE ORIGINAL APPROVED PLANS AT THE JOB SITE, WHICH HE DOES. DAVE HATTRICK SAID TO JUST ADD POND'S INC NAME TO THE PERMIT AND MAKE IT THE PRIMARY CONTRACTOR, WHICH HAS BEEN DONE. THE REMAINING WORK CONSISTS OF REPAIRS IN 4 OF THE 5 BATHROOMS (UNISEX BATHROOM IS COMPLETE).

It was brought to my attention by Sue Millard, front desk personel of Building Department, that the contractor has changed for this project. Gilbert Pond, Pond Inc., has taken over the project from J & G Construction. I discussed this with Marlene Southard, my immediate superviser. It was determined that we would need to determine who paid for the original permit, the contractor or the tenant to find out if a refund was due to the contractor, J & G Const. if they paid for the permit. If a refund was given, then Pond Inc. would have to pay the permit fee portion returned to J&G Const. in order to be put on the permit as the new contractor. If the tenant paid for the permit it was felt that the new contractor could just be added to the existing permit. I explained this to Sue. Sue informed me that her supervisor, Dave Hattrick, had instructed her to just add the new contractor to the permit and make an action entry in the computer of what had taken place. Any alteration of fee's would then be handled if an when the original contractor came in to request a refund, if he paid the permit fee's. I explained this procedural direction to Marlene. The change was made to the permit by Sue Millard as she was instructed. drs

Phone call to James Kreller, contractor to verify that there was not going to be any changes to the existing doors from the corridor to the remodeled bathrooms. This project is a result of deterioration of the toilet/shower room interiors because of water and use. Repair project will include the upgrade of toilets and showers to be usable by the elderly and disabled per ADA requirements as directed by the architect, John Cook.
