Description	Scope of Work GLENWOOD MANOR - To install a (modified) life safety NFPA 13D system in an existing six bedroom, three baths, 2,278-ft/2 one story intermediate care dwelling. The modification is the installation of fire sprinklers meeting Idaho State Facility Standards with sprinklers installed in all closets and restrooms. System shall be equipped with an alarm initiating device and an outside alarm. All fire sprinkler work and related, which includes, single service water supply, the system backflow prevention device shall be installed or under the license by a Fire Sprinkler Contractor in the State of Idaho, which shall be installed per NFPA 13D, approved plans, engineering/red-lining and attached review comments, under direction of the Fire Chief. This permit is not attached to any building permit.

Several items need addressed, left a 6 item c/n on site including heads installed do not match the type system was designed for or submitted on, the single service supply as indicated has not been witnessed for flush see note #11, need to provide access to inspect the OH installation as no rough-in inspection was requested, there is a non-monitored shutoff valve upstream of the system controls and flow switch see note #13 for requirements, the WH closet needs an intermediate head installed, provide verification of the type of antifreeze used as this system is CPVC. NOTE: the sprinkler monitoring has been connected without a fire alarm permit.
