Fire hat functionality both intermitent and solid tested satisfactory for all four elevators, final pending reciept of NFPA record of completion documents.

Reviewed and reinspected defficient items noted at the previous inspection including elevator shunt trip power supervision, recall for car #3, new FJ at the pump room, new antifreeze system tamper, flow switch at the NE 2nd floor stairway, tampers at riser and floor control valves. Two remaining items in need of repair include the 5th floor NE tamper at the OS&Y sprinkler control valve, and provision for a flashing fire hat at the elevator cars upon FA device activation in the hoistway or machine room.

Inspection for elevator recall including testing lobby smoke detectors at all levels and elevator hoistway/ machine room devices. Noted shunt trip power supervision circut 20 address 122 did not report, SD 2031 is incorrectly located on the map (needs to be shown in the elevator machine room), the southeast car (elevator #3) is out of service-will need to verifiy recall at next visit, per ANSI/ASME A17.1-2000 Safety Code for Elevators- fire alarm initiating device activation in a machine room or hoistway shall cause the fire hat visual signal within the car to illuminate intermittently (2.27.3.2.6), noted SD locations at each of the four hoistways does not appear to meet requirements. Also noted spot type smoke detectors at the mechanical penthouse are not in compliance with NFPA 72 2002 sec. 5.7.3.2.1 and 5.7.3.2.4(B)(2) regarding mounting locations, beam obstructions and spacing 5.7.3.2.3 (A).

Fire alarm testing for the 1st floor SE quad as annotated on approved plans is satisfactory.

Fire alarm system testing; Saturday April 28, 2007 -Fail

1. HVAC Fan control from Fire Command Center tested Start/Stop - Pass
2. Stairway pressurization control from FCC tested Start/Stop - Pass
3. Plaza level North & South automatic door opening tested for smoke control - Pass
4. HVAC system status indicators are switch position indication only and are not true air flow or motor running status. Must be determined by Dave Hannemen.
5. Tested duct smoke detectors and AHU shutdown of basement air handlers AHU11, AHU9, AHU12, AHU13, AHU10 and AHU15. All duct detectors shut down units OK. However they are located in the supply air ductwork. Per the upgrade agreement and IFC 907.2.12.1 Duct detectors shall be located in the return air ducts. - Fail
6. Descriptor for basement FACP room smoke detector needs to be corrected. - Fail
7. FACP panel smoke detector to close to the diffuser must be relocated - Fail
8. Remove old non working fire alarm devices - Recheck as project progresses
9. Fire phone system not operational - To be tested as project progresses
10. Elevator annunciator panel does not indicate elevator floor locations - Install correct annunciator
11. Guard station annunciator not operational, paging system in place - Needs to be demonstrated.
12. One atrium exhaust fan wired incorrectly for remote operation - Corrected in place OK
13. Building dampers that isolate building penthouse HVAC control systems from the atrium smoke control removal did not operate. The control relays energized but dampers could not be shown that they would be overridden in an automatic smoke control sequence - Fail
14. Sprinkler system Flows & Tampers not tested. Need to be demonstrated
15. Several covers not installed on the new fire alarm control relays penthouse - To be corrected
16. Note: Cannot sign off Fire Permit Fir06-00595 until all floor level core areas are complete basement through penthouse see approved plans.

Pending correction of programing issues for descriptors and device address numbers matching an approved plan set.

Inspection for the 5th floor Office supply tenant alarm work failed due to non-functional speakers at several locations, two SD have incorrect descriptors, as-built plans should reflect field changes, the notification and initiation devices at the north hall are not installed at this time. Also verified added SS at the mech. penthouse. Attempted test of the 1st floor atrium doors failed due to non-functional manual pull station at the reception desk.

Partial alarm notification testing completed for the 4th floor SE and SW quads and the entire penthouse. Noted two non-functional pull stations and one non-functional strobe at the 4th floor, still need to test the SD in the electrical and telephone rooms, the penthouse is missing one strobe to be added, one a/v is not functional on audio, only one manual pull tested at this time.

Inspection for the basement level audio/visual alarm notification functionality and coverage only completed today, noted file room coverage is lacking, still need to test manual pulls, relocated SD's, check EOL voltage.

2nd floor SW quad alarm test completed satisfactory.

After hours Inspection Boise Cascade Building. Fail

1.) The installing contractor is using a non- approved set of floor plans for installation of fire alarm equipment on the 4th floor. The approved set of plans was found in the general contractor's office, and after review the installation was significantly different than what was on the approved set of plans.

2.) Began by testing a smoke detector outside of the Northeast stairwell, found that an alarm verification feature is programmed into the automatic fire alarm detection devices. The main FACP descriptor associated with the smoke detector had only confusing identification numbers with no association to the floor or area of the detector location.

3.) Tested another smoke detector outside of freight elevator, alarm verification was enabled on this detector also. Finally after the detector went into alarm the smoke guard doors released and another confusing descriptor displayed at the main FACP. This time the descriptor said that the alarm was at the plaza level elevator smoke detector.

4.) It was decided that a meeting needs to be set up to discuss the procedure for installation, testing and correcting problems found during inspections. Also determine why installing workers are not working from approved plans.
