Fire alarm system upgrade to include relocating manual pull stations to required ADA height, relocate firefighters emergency phone system jacks to inside adjacent stairwells, installation of new audible visual fire alarm notification devices, installation of new fire alarm power supplies and transponders on each levels indicated, installation of a stairwell pressurization system. Repair & installation of atrium smoke exhaust system, installation of smoke guard doors on freight elevator all levels except plaza level, and the installation of a new fire command center located on the exterior west side of the building. Beam detectors are shown to be added protecting the plaza level Atrium & walkway. The remodel work will occur in an existing 322,795 sq. ft. high-rise building that has a 77,950 sq. ft. basement to comply with some of the upgrade work outlined in a 4/17/06 Upgrade Agreement on file for this building. Upgrade work will include installing a complete fire sprinkler system throughout the building, removing existing sprayed fire-resistant materials containing asbestos and providing new sprayed fire-resistant materials, providing at least one accessible Men's and Women's restroom on each floor, Special Inspection required for sprayed fire-resistant materials and the smoke control system. This is a Scope of Work Permit. A Letter of Completion is to be issued when done. NOTE: This building will be fully fire sprinklered under this permit. JCB

Noted shunt trip power supervision circut 20 address 122 did not report, SD 2031 is incorrectly located on the map (needs to be shown in the elevator machine room), the southeast car (elevator #3) is out of service-will need to verifiy recall at next visit, per ANSI/ASME A17.1-2000 Safety Code for Elevators- fire alarm initiating device activation in a machine room or hoistway shall cause the fire hat visual signal within the car to illuminate intermittently (2.27.3.2.6), noted SD locations at each of the four hoistways does not appear to meet requirements. Also noted spot type smoke detectors at the mechanical penthouse are not in compliance with NFPA 72 2002 sec. 5.7.3.2.1 and 5.7.3.2.4(B)(2) regarding mounting locations, beam obstructions and spacing 5.7.3.2.3 (A).

4. HVAC system status indicators are switch position indication only and are not true air flow or motor running status. Must be determined by Dave Hannemen.
5. Tested duct smoke detectors and AHU shutdown of basement air handlers AHU11, AHU9, AHU12, AHU13, AHU10 and AHU15. All duct detectors shut down units OK. However they are located in the supply air ductwork. Per the upgrade agreement and IFC 907.2.12.1 Duct detectors shall be located in the return air ducts. - Fail
6. Descriptor for basement FACP room smoke detector needs to be corrected. - Fail
7. FACP panel smoke detector to close to the diffuser must be relocated - Fail

Attended an inspection conference with Romar Electric, to discuss the location and mounting height of the Boise Cascade atrium smoke beam detectors. It was agreed that the contractor would locate the beam detectors as high as physically possible above the horizontal floor of the atrium. The new beam detectors will be spaced per the manufactures recommended spacing 30 - 60 feet. It was agreed that 30 foot spacing would be acceptable between the beam detectors and half of that distance would be allowed to a side wall of the atrium see manufacturers cut sheets.

1.) The installing contractor is using a non- approved set of floor plans for installation of fire alarm equipment on the 4th floor. The approved set of plans was found in the general contractor's office, and after review the installation was significantly different than what was on the approved set of plans.

2.) Began by testing a smoke detector outside of the Northeast stairwell, found that an alarm verification feature is programmed into the automatic fire alarm detection devices. The main FACP descriptor associated with the smoke detector had only confusing identification numbers with no association to the floor or area of the detector location.

3.) Tested another smoke detector outside of freight elevator, alarm verification was enabled on this detector also. Finally after the detector went into alarm the smoke guard doors released and another confusing descriptor displayed at the main FACP. This time the descriptor said that the alarm was at the plaza level elevator smoke detector.

4.) It was decided that a meeting needs to be set up to discuss the procedure for installation, testing and correcting problems found during inspections. Also determine why installing workers are not working from approved plans.
