This fire alarm permit is to add speakers and wiring for potential future strobes per IFC 907.10.1.4 as an extension of the existing fire alarm system. No new initiating or visual devices are part of this permit scope.
This is a modification to the shell permit.

Testing completed for the remainder of the new elevator control devices including heat detectors at 3rd and 4th floors and elevator machine room, (damaged heat detector at the 4th floor replaced while on site), shunt trip and voltage monitoring, retested non-functional smoke detector at the 5th floor elevator lobby (replaced while on site), 9th floor flow switch retested, 5th 6th and 7th floor in duct detectors for fire smoke dampers, tested all smoke alarms at residential units on the 8th and 9th floors. NOTE: Need to provide sprinkler monitoring for the exisitng system at the adjacent parking structure per TOC agreement letter item #6/7, non-functional smoke alarm at unit 904, plans need to show accurate locations of devices at the basement, 1st and 18th floors (flow switch, tamper valves, SD and HD), post map at the fire command center, approved plans to reflect scope of work completed-new devices and interconnection to to systems at the adjacent building are not installed at this time, provide record of completion and inspection and testing reports.

Completed smoke alarm testing for all units at floors 5,6,and 7, primary and alternate floor recall for the new elevator including fire hat functions tested okay, 4th floor fire alarm testing passed. NOTE: Missing equipment to test the heat detectors at the 3rd, 4th and elevator machine room, shunt trip and voltage monitoring also needs tested, smoke detector at the 5th floor elevator lobby failed to operate, plans indicate heat and smoke detection in the hoistway which is not installed-devices are located in the machine room, need to locate duct detectors for testing at the 5th, 6th and 7th floors, flow switch at the 9th floor needs repair/retest.

The items inspected today were the firemans phones. The phone operated except the 15th floor east which would cut out and needs replaced. Also phone jacks are shown to be installed on the exterior stairways but have not been installed.

Items tested today include the remainder of manual pull stations on floors 18 thru the basement with the exception of one MP at the 1st floor west side tenant space and two MP at the 18th floor, no access to test the 4th level parking garage due to floor coating, audibility retest at the common corridors and condo units completed satisfactory, tampers at the floor control valves 5th -18th post relocation tested okay, 14th floor corridor speaker strobe repaired, new strobe at the fire command center tested ok, need access to the communication panel for fire phone testing, 11th floor speakers at units are not ready for testing due to construction underway. Need to post an accurate building map at the panel, provide new plan sheets for the basement and 1st floor device location changes for tamper and flow switches..

Walkthrough completed for the basement, 1st, 2nd and 3rd floors. Noted plans indicate 18 ah batteries( 7ah are installed) , plans show a nac and one sd in the basement electrical room which are not installed, notification devices at the basement corridor are wall mount type positioned on the ceiling, scope of work per approved plan is not finished for interconnection to the exitsing adjacent builidng alarm devices, noted smoke alarms and audible notification at numerous residential units are incorrectly located per NFPA 72 guidelines, missing lockouts at breakers to the FACP. NOTE: Phone lines, power loss, battery trouble, smoke above panel tested okay. Fire pump and all sprinkler system monitoring points for the basement, 1st and 2nd floors completed okay-need to show accurate locations of basement backflow valves and flow switch and ISO vavles for the standpipe on plans and map. Inspection completed 4/13..
