The north duct smoke detector in the Billiards store did not shut down the hvac unit as per chapter 6 of the IMC. The other duct smoke detectors operated correctly.

The first time I stopped at the project the facp was not dialing out to the monitoring station. I returned acouple of hours later and the problem, wiring related, had been fixed. I verified signals with the monitoring station. I also pointed out that the manual pull station was mounted too high and the technicians had lowered at by the time I arrived for my second inspection. I notified sam rogers and mark sentenno.

Sam,
Thank you for issuing the quick permit FIR08-00237. I contacted Bill Herbert (General Fire) on Thursday the 3rd, he stated they were working on the job. I attempted to contact Bill this afternoon to confirm they had completed the job and were ready to call for "verification of functionality". Mike (General Manager, General Fire) stated that Bill no longer worked at General Fire. I requested that he track down the information and get back to me this afternoon, he stated he would call me later. Therefore, I explained to Mike that Bill had hired me to do the submittals for this job and I arranged the agreement to allow the panel to be replaced under the constraints as indicated in the permit.
I apologize that my client did not keep their end of the bargain,
hopefully Mike will get this pulled together and we can get the
verification accomplished and I receive the as-builts to get plans in
for the permit.

I called Mike (Jr) (General Fire, ###-####) regarding current status. He indicated they installed an SK FAC panel on 4/2 and restored it to functionality. He indicated they put in a different panel (a Silent Knight) than originally quoted, to expedite replacement. They did not schedule a PDS/Fire inspection to verify "functionality", as stipulated in the "Quick Permit". Installers are on site now, changing existing 18g wire to 14g to the existing horn/strobes but no information or plans have been given to M. Froemke to prepare a new system design. He was unaware that a panel replacement necessitates visual coverage be provided accordance NFPA 72 requirements (IFC 907.10.1) so they have not evaluated the existing coverage. He agreed to: 1) immediately schedule an inspection for a "functionality" test to be done this afternoon; 2) make an onsite visit to evaluate the existing system today: 3) provide a building floor plan and details on the existing initiating devices and notification appliances, the new FAC panel and the wiring modifications they have made, to M. Froemke so she will be able to design the new system immediately, and have the system ready for final inspection by 4/16 as stipulated by the issued permit.

Mark Senteno called the building owner, Sundance Properties, Jerry, ###-#### expalined the concern to get the fire alarm panel back in service quickly or a fire watch may be required.

Per agreement with Mary Froemke, designer, 4/1/08. This is a quick permit preliminary approval to allow emergency replacement of the existing defective fire alarm panel, to reinstate sprinkler monitoring. Preliminary inspection to be scheduled today to verify monitoring is in service. Plans are to be resubmitted for approval and final inspection within two weeks.
