11/16/10 PLAN MODIFICATION (12 initiating devices affected):
This plan modification is to relocate an existing pull station, add pull stations at new exits, and extend the existing corridor automatic smoke detection into the new areas. In addition, this plan modification is to incorporate previous review comments and field modifications.

10/11/10: This fire alarm permit is to provide notification and a duct detector for HVAC shutdown/monitoring due to a tenant improvement. New duct detector to be connected to the existing FACP. A new NAC panel is being provided for the new notification devices.

Associated building permit: BLD10-01201 (ST. LUKE'S Orthopedic Clinic - American West Bldg - Ste # 120) Permit is to remodel an existing 6,300 square foot portion of an existing building. The 6,300 square feet is a combination of the area left over after BLD10-01138 and Pioneer Title & Escrow (BLD05-03033). Approval contingent upon adhering to the approved plans on file. Work to include: [construction of interior non-bearing walls to create separate sex accessible bathrooms, two unisex accessible bathrooms, 12 exam rooms, 4 offices, a reception / waiting area, a new vestibule, 5 storage rooms, a nurses station, and a wet clean room], installation of new plumbing, installation of new lighting, installation of new mechanical, installation of new doors / hardware, and finishes in compliance with chapter 8 of the 2006 IBC. This building is fully fire sprinklered any alterations or modifications to the sprinkler system WILL REQUIRE A SEPARATE PERMIT and shall be installed per NFPA #13 as regulated by the fire department. Racking that exceeds 8 ft. in height requires a separate Building Permit be submitted and approved before installation. **TLW**
