The 2" poly piping needs to be attached to the bottom of the trusses, need the city approved plans to verify the riser layout and sizing, need the flow switch/bell repaired. I spoke to the owner(husband) about the need for more fill dirt over the underground per the agreement with the fire sprinkler contractor and builder.

City approved drawings were not on site at time of inspection. Flow switch fire bell was not installed at time of inspection, fire plan review notes specify that the bell needs to be mounted on an exterior wall, close to the street address side of house. Numorous concealed type escutcheons are painted, all escutcheons that were painted after installation need to be replaced. Point of contact on project did not know where the underground supply came into the building, previously noted item for proper coverage above underground supply could not be verified, this will need to be addressed/verified before next inspection.

Per our phone conversation, please see attached photo regarding the final grade at the slack residence. Per our discussion, during rough in inspection the grade datum that was used was where the current grade was 12" below the concrete stem wall and the pipe was reported as being 3" too high.

Mussell Construction confirmed at that point that grade would be raised 6" plus throughout. Please see attached photo that shows the finish grade to be approximately 4"-5" below concrete stem wall. Thus indicating that grade has been raised as previously indicated. Please concur with this information.

I called and left a message with point of contact Scott Phone #: ###-#### , appears that a SUEZ representative is not scheduled to perform flush (I spoke to SUEZ and it was not on their schedule), main line underground flush will not be completed until a SUEZ representative is schedule to operate underground valve to perform mainline flush. This inspection request is cancelled and a new underground flush request will need to be rescheduled with a SUEZ rep to operate valve.

This inspection was for an underground cover. Piping is installed at the other end of the house and is not at the required 4'-6" minimum bury depth. Piping can not be covererd.
