On Friday the 22, Larry Sundell turned plans into the permit desk for processing. He was given a BLD number and paid required fees. He turned the plans over to Plan Review Technician Diane Lewis. She was instructed by Marlene to hold them until I returned back to work Monday ( I was off Friday). BLD02-00365 for interior finish and code upgrade is in conjunction with this MIS permit. Both have to be finaled in order for the tenant to obtain a certificate of occupancy in order to open for business.

Larry Sundell called and came in to pick up the file he had previously left. He wanted to review the interior details and floor plan he had drawn to show the location of the foam plastic. He also wanted a copy of ER-5161 to use to provide installation details & requirements for the job. The tenant had decided that they would remove the foam plastic allready installed and then reinstall it according to the ER-5161 report wiht the FC-90 as the adhesive beteen the foam plastic and the sheetrock and then to encapsilate the remaining exposed foam. I reviewed Larry's plan and told him the plans would need to indicate what the thickness of foam would be; the maximum dimensions of each foam piece per ER Report and any other items that need to be corrected or constructed to allow the FC-90 product to be used per the ICBO ER-5161 report. I also told him that 20% ($1,500.00) of the project cost, ($7,500.00) would need to be used toward accessibility upgrades per Appendix Chapter 11, 1997 UBC and this would need to be clearly indicated on the plans.

Larry Sundell came in with application, and pisture's of the interior walls & ceiling showing the foam plastic. There was a lot more EPS installed than had been previously understood in discussion with tenant. Larry left the application and folder with picture's for consideration by staff.

Tenant Mahmoud Al-Hatab & Petros Pafias brought in stop work order and description of what they had done. They were told they would need to remove the foam plastic or come up with an approved product that could be used to provide the required thermal barrier to separate the foam plastic from the interior. I gave them a copy of ER-5161, FC-90 Foam Coat as an example of a product that may work. They were told that they would need to obtain an Idaho licensed architect and provide plan details for the interior finish remodel if they intended to leave the foam in place.

Alan Hauser performed a mechanical inspection at the job site for replacement of a RTU. Carl Dependahl had been notified by a plan reviewer that the Bedrock Cafe was applying a foam plastic interior finish outside the scope of the MIS01-00151 permit issued 8/1/01 for an occupancy approval review for which no inspections had been called. Carl asked Alan to issue a Stop Work Order and post it at the job site. Alan did this.
