I VISITED WITH BEV DUGGER, PW TO SEE IF, TO SEE IF THERE ARE QUESTIONS CONCERNING THE TENANT CHANGE WITH SAME OCCUPANCY USE. SHE ASK THAT I GIVE HER A COPY OF THE MENU THAT MS. FREIDBERG IS TO FAX ME. I LEFT A COPY AT HER WORK STATION FOR HER FILE.
I CALLED CDH AND TALKED WITH JANINE. LAST COMMENT IN FILE ON INSPECTION CALLED FOR CUTTING BOARDS TO BE SANDED BY 6/2002. CAROL JOHNSON IS FIELD INSPECTOR FOR CDH AND SHEE IS OUT ILL TODAY. OCCUPANCY APPROVAL FOR C.O. IS CONTINGENT ON CDH APPROVAL.

Met with Jeanette Freidberg to review process to obtain a new C.O. as she is the new tenant owner and the sandwich shop will obtain a new name, Kimo's Subbies. I gave her and MIS Application to fill out and ask her to fax me a copy of her menu so I could verify that PW & CDH did not have any issues that she would need to address. After this is verified I complete and MIS Permit for an inspection and sign off by CDH, fire & structural inspectors. When approved by all three inspections agencies a new Certificate of Occupancy can be issued. If inspection reveals changes have been made since the 1995 plans for Quizno's, tenant will be required to provide additional information as noted by the inspectors correction notice issued at time of inspection with additional application, plans and permits as required to be provided.
