Obtained written files for BLD98-02375 and copies of red-lined floor plan showing changes that were to be made on originally approved plans in our Records Div. as is currently allowed under Boise city PDS Dept. Policy. Prepared permit description and copies of past plans and data for use by the field inspectors to perform the Occupancy Approval Reveiw.

Called Lisa Arnold, ###-####, LB Industries, Inc. dba ISAIAH 61, TO INFORM HER THAT THE PERMIT COST WOULD BE A MINIMUM OF $132.30 AND ASK HER IF SHE STILL WANTED TO PROCEED TO GET THE PERMIT. I left a phone message as she was not immedialtely available. Lisa called me back an instructed me to proceed with the permit and she would arrange for the additional fee payment when she came in to pick up the permit.

Reviewed status of application with Marlene Southard, Plan Review Div. Manager. BLD98-02375 had been applied for on 8/4/98 and issued for T.I. on 8/6/98. There are no records of calls for or performance of any inspections by the structural division. An electrical permit issued in conjunction with the BLD permit, ELE98-02774 was issued 8/13/98 and finaled on 9/22/98.
Marlene instructed me to create the permit but that the fee's would be based on the time spent in researching and preparing the permit; time involved for field inspection and time involved to create and record the certificate of occupancy once approval from field inspectors is obtained by applicant. This was determined to be three hours @ $44.10 = $132.30.
