(SHAW MOUNTAIN SKILLED NURSING FACILITY – PASSENGER LOADING ZONE AND CANOPY – NORTHWEST CORNER OF BUILDING – REPLACEMENT OF ALL WINDOWS IN THE BUILDING) (10-2-17 PLAN MODIFICATION TO INSTALL A FOUR LEAF DOOR – TWO LEAVES SLIDING WITH ALL FOUR LEAVES ABLE TO BREAKAWAY IN THE DIRECTION OF EGRESS TRAVEL – AND TO RECONFIGURE THE RAMP AND RAMP LANDING AT THE MAIN ENTRANCE TO THE FACILITY. THE CROSS SLOPE OF THE RAMP MEASURED PERPENDICULAR TO THE HANDRAILS WHICH IS ALSO PERPENDICULAR TO THE DIRECTION OF TRAVEL WHILE USING THE HANDRAILS IS NOT ALLOWED TO EXCEED A 2% SLOPE.) The fire sprinkler system is used for occupancy and for building area increase. NFPA 13 does not require the new canopy or covered ramp to be protected by fire sprinklers if the construction is entirely noncombustible. To remodel an entrance to an existing Skilled Nursing Facility, construct a 1,094 square foot covered passenger loading zone addition, construct a 200 square foot covered concrete ramp addition, and to replace all exterior windows in the building. The total addition has a floor area of 1,294 square feet. Demolition work includes removing exterior brick screen walls, exterior concrete sidewalks, exterior windows, exterior door, and an entry canopy. New work includes a new 1,094 square foot canopy with a membrane roof covering over a passenger loading zone (reinforced 4 feet diameter by 17 feet deep concrete footings, steel columns, steel beams, steel beam to steel column moment connections, steel roof purlins, fire-retardant-treated wood roof deck), a new 200 square foot canopy with a steel roof covering over the new entrance ramp (reinforced 1.5 feet diameter by 2.67 feet deep concrete footings, steel columns, steel beams, steel roof deck), a new concrete ramp with a handrail on each side, a passenger loading zone, new concrete sidewalk on grade, and all new windows for the entire building. Special Inspection is required for intumescent fire resistive coatings, soils, concrete, cast-in-place deep foundations, and steel (including welding). The work is required to comply with the approved drawings, with the structural design, with the plan review report, and with all applicable codes and ordinances. The fire sprinkler system is required to comply with NFPA 13 and the fire alarm system is required to comply with NFPA 72 as regulated by the Fire Department. Separate fire sprinkler permits and fire alarm permits shall be obtained prior to beginning any modification or installation work on such systems. **CJW**
