ALPHONSUS – 2ND STORY – BELOW CENTRAL TOWER) (4-4-18 PLAN MODIFICATION TO PROVIDE MORE LEAD SHIELDING, REVISE DOOR LOCATIONS, RE-ROUTE DUCTWORK, RE-ROUTE MEDICAL GAS PIPING, REMOVE SOME MEDICAL GAS PIPING, AND TO MAKE CHANGES TO ACCOMMODATE EXISTING CONDITIONS.) The fire sprinkler system is used for high-rise and for occupancy. To remodel three areas on the 2nd story having a total floor area of about 2,043 square feet to be used as an endovascular operating room (EVOR), anesthesia supply and work room, and anesthesia offices. Work is to include interior noncombustible nonbearing walls, lead shielding, suspended acoustical ceiling, gypsum board ceiling, medical equipment, a floor bearing plate to support medical equipment, overhead structural support grid for medical equipment (horizontal Unistrut for gravity loads, vertical Unistrut for gravity loads, and diagonal Unistrut braces for seismic loads), new doors, counters, cabinets, and finishes. While transporting the new medical equipment through the building and while staging for installation, floors and elevators are not allowed to be overloaded. Special Inspection is required for welding, for bolting, and for post installed anchors in hardened concrete. The installation and use of X-ray equipment, including shielding at new and existing walls, is required to comply with all applicable Industry, State, and Federal regulations. The work is required to comply with the approved drawings, with the structural design, with the plan review report, and with all applicable codes and ordinances. The fire sprinkler system is required to comply with NFPA 13 (including the installation of quick-response fire sprinkler heads within altered areas) and the fire alarm system is required to comply with NFPA 72 as regulated by the Fire Department. Separate fire sprinkler permits and fire alarm permits shall be obtained prior to beginning any modification or installation work on such systems. **CJW**
