Description
To add additional tamper switch monitoring to the sprinkler system. The existing fire alarm system is a required sprinkler monitoring that includes door release and partial area coverage.

12/14/2004 - Revised scope, the original fire alarm control panel was replaced with a Firelite MS-5012. An ESL 505 module will be used for connecting the ESL 429CT smoke detectors (compatibility listing is included in the packet). The battery calculations indicate a minimum battery size of 9.73 a-h, the 12V FACP will hold 7 a-h batteries. The installed battery size is not defined and will be field verified by the PDS Fire Inspector. The revised fire alarm matrix indicates that on alarm, the FACP will "unlock exits", but no output from the panel is shown to be connected to the exits for release. The PDS Fire Inspector shall field verify the FACP releases the exits upon alarm.

1/19/2005 - This plan modification defines the new FACP (Fire Lite MS 5012) that has the same functionality as the previous panel and is compatible with the initiating devices already installed except that it has "latching" supervisory signals.

NFPA 72 (2002) specifically addresses both non-latching and latching supervisory signals (4.4.3.2.3 and 4.4.3.2.4). Per NFPA 72, 6.8.5.7.5, "Where approved by the AHJ, supervisory signals that latch in the off-normal state and require manual reset of the system to restore them to normal shall be permitted." Dave Miller, Boise City Fire Marshall, has agreed to this panel being installed.

This plan modification also addresses the following:

" Two 12v, 7 a-h batteries will be installed in the 12v system to meet the calculated a-h capacity.
" West side of main lobby, seven (7) smoke detectors that were originally installed for fire/smoke door release are being removed, the two (2) roll-up fire/smoke doors are deactivated in the OPEN position and the door hold-open on the northwest hall will also be deactivated.

The PDS Fire Inspector shall field verify battery monitoring capabilities, removal of smoke detectors and the position of the fire/smoke doors.

The associated building permit (BLD 03-01575) is to construct 3 offices and a conference room within an existing 12,213 square foot medical office building. Two existing restrooms are also being altered to be accessible to the disabled. Work is to include interior nonbearing walls, interior doors, interior windows, and suspended ceilings all per the approved drawings and plan review conditions on file. The location of fire sprinkler heads shall comply with NFPA 13-1999 as regulated by the Fire Department. **CJW** (SELAH MEDICAL CENTER) This building is fire sprinklered to allow non-fire-resistance rated corridors.

Battery calcs, flow switch, battery supervision and smoke detectors on the east side of building (5 total) operate correctly. Inspection stopped after finding the items listed. The monitoring company is not getting all of the signals sent such as Phone line 1 and low battery. Smoke detectors on the west half of the building annunciate locally but do not report to the panel. The tamper switches supervisory signal is latching and is not supposed to per nfpa 72 4.4.3.2.4. Per the manufacturer the panel can't be reprogrammed. 12/17/2004 1:56:55 PM

Inspection was stopped due to so many items being incorrect. Phone lines are not being supervised. Smoke detector installed is not the one submitted. Supervisory signal latches. No map of initiating devices. Need zone description at annunciator. No one to test the new tamper switches. Strobe does not work.

At Mary's request I stopped at this project to verify if smoke detectors are installed and if possible the reason they are installed. I found smoke detectors installed on the left hand side of the lobby at three fire doors and what appears to be area coverage through out the office area to the right of the lobby. The smoke detectors at the fire doors are mounted on the walls on the lobby side and appear to exceed 12" down from the ceiling
