Description
This permit is to tie in a waterflow and tamper switch located in the 6th floor stairwell heading up to the penthouse, and tie in a kitchen hood to the fire alarm system. Alarm vendor indicated that the flows and tampers must have been installed after the building sprinkler system was installed. However, no sprinkler permit was found for the installation of this flow and tamper switch. The flow switch and tamper switch need to me monitored by the building fire alarm system. An MIS permit MIS05-00136 was issued and finaled for the Bombay Grill. This permit includes connecting a kitchen hood in the Bombay Grill to the fire alarm system. Two kitchen hood permits were found FIR06-00029 and FIR06-00593. Neither permit indicates which restaurant or tenant improvement they were for.

Site inspection comments per K.Roule
Tested FA monitoring of VT and waterflow (by manual means). Both annunciated correctly at panel and offsite monitoring station. Visually observed location of dry sidewall sprinkler installed in exterior chimney. Work remaining on this permit:
1. Test the new fire alarm monitoring module which was installed on the existing cooking hood suppression system under this permit. (Requires the hood fire suppression system contractor to be present)
2. Test the waterflow alarm switch by flowing water from the test connection. (Requires the sprinkler contractor responsible for FIR04-00490 be present).
3. Provide fire alarm-initiated interlocks which shut off the hood exhaust fan and the gas fuel to the cooking appliances, as specified in the signed Fire Department Agreement dated 2/4/2004 and in F.Varley's letter dated 4 Feb 2002 letter (Item 2).
4. Correct the existing "trouble" condition on the fire alarm panel.
5. Provide an updated alarm map reflecting the current floor plan and all initiating device locations correlated with the annunciated fire alarm device addresses. (NFPA 72 4.4.6.1).
6. Replace the missing door latch on the FACP cabinet and update the outdated monitoring company and account information currently posted inside the cabinet.
