6-18-09 PLAN MODIFICATION:
This plan modification is to install notification and initiating devices in new construction and a remodeled area. Duct smoke detectors have been added for new AHU's. The single station smoke detectors used for door holder release are programmed as supervisory. This is allowed only due to the door holders not being used for smoke control in corridors. Addressable relays have been added to control maglocks installed by others.

This fire alarm permit is to install additional notification and initiating devices to accommodate the addition. These devices are to be added to the existing fire alarm control panel.
Associated building permit: BLD07-01846 (VALLEY VIEW - SKILLED NURSING FACILITY) To remodel select portions of the existing interior and to add to the exterior of an existing skilled nursing facility. Work included in this permit as follows: Extending the existing two hour fire wall, which is used to separate this building from the R-2, independent living facility and is also used to separate the two different types of construction. To add approximately 5,810 square feet of new floor area to the first floor, this tabulated area includes a 3,800 square feet of additional floor area for the physical therapy wing, enlarging the existing dining room by 1,120 square feet, and the remainder 890 square feet includes the new covered entries and storage rooms. To add approximately 2,600 square feet of new floor area to the second floor. This area includes a 1,120 square feet addition to the existing dining room and 1,455 square feet is for new office space above the new physical therapy wing. Additional work included in this permit is to renovate selected residence rooms, office spaces, the nurse's station and lobby areas. All construction shall be per the approved plans and attached comment list. NOTE: This building is fully fire sprinklered with a NFPA 13 fire sprinkler system and is used for area increase. Area in question is at the main entry lobby. Smoke detectors for door holder shutdown have been relocated from original installation and are not part of the submitted scope of work. No inspection performed. Spoke to contractor about what would be required for submitting the non-scope changed area. Also discussed requirement for submitting a plan modification for changes within the scope of work but are too extensive for field approval.

See the city approved plans for deficiancies. They include smoke detector and a door holder that are not shown on the plans. A door holder that is shown on the plans but has not been installed. A remote annunciator that has not been installed. A smoke detector that does not set off the general alarm. A room missing a strobe. A wall mount a/v has been revised to a ceiling mount and was the wrong candella. All of these items are in a very small area.

The fire alarm systems in the first and second floor dining areas operated correctly. As-built plans will be required at a minimum or possibly resubmittals depending on how many other changes are made to the project. Just in these two areas the a/v's are ceiling mount instead of wall mount, duct smoke detectors have been added, spot type detector has been added for hvac shutdown and the wall layout has changed. Also the spot type detector being used for hvac shut down has been installed out side of its listing as far as max. cfm and the physical positioning within the duct opening.
