5-20-09: PLAN MODIFICATION
This plan modification is to indicate addressable notification devices (as opposed to the standard shown on the original submittals), to implement changes due to the previous review comments, to remove the heat and smoke detection in the elevator shaft (a sprinkler head in the shaft was not installed), revision of monitored sprinkler devices, and addition of a heat detector in the elevator mechanical room. Also included in this modification are the correct data sheets for the LCD annunciator and smoke detectors (previously submitted on incompatible parts for these devices.)

10-13-08:
This fire alarm permit is to provide a new FACP for sprinkler monitoring, elevator recall, duct detectors, and a manual fire alarm system for the core and shell of the new building. System to include manual pull stations and notification in the common corridors. Audible notification has been provided in specified rooms; however visual notification and smoke detection shall be added during future tenant improvements under separate permits.

Associated building permit: BLD08-01210 St. Alphonsus - MOB VII shell & core building - To construct a new 70,705 square foot, three story, steel frame and concrete medical office shell and core building related site improvements. The core areas include stair enclosures, elevators, mechanical and electrical rooms and entry lobby. This building is fully fire sprinkled. The sprinklers are used for area increase. Any I-2 Occupancies would be limited to the first floor and must be separated from the remainder of the building by minimum one-hour fire-barriers.
