Description
This project is for the shoring and excavation for the Shipping/Receiving building. Refer to the St. Luke's Masterplan for more information. Single master SWPPP created for both the shipping/receiving building and the NW Parking Garage. See ESC18-01167 for parking gararge. BMPs include pollution prevention practices in staging areas and impervious areas, sediment protection on a receiving storm drains. If any dewatering is required, a dewatering plan must be submitted to ESC division for review and inspection. Site preparation /pre-construction conference is required.

- stormwater controls to direct stormwater from perimeter to within site for retention and limit offsite discharge.
- access points are swept at minimum once per week. No trackout or dirty surfaces observed at site access points.
- concrete washout pans located in multiple locations on site and being serviced at time of inspection
- secondary containment for mixers at the shipping and receiving site
- working on removing dirt and debris in gutters in areas where fencing is tight and difficult for cleaning. On 2nd street will contain both ends of gutter with sand or rock bags
- suggested installing a gatorguard wattle at Bannock that can contain the dirt at the access area but also be driven over by trucks (should be removed for heavy use or tracked equipment)
- suggested installing secondary contaiment for dumpsters or cover to prevent waste mingling with stormwater or leakage

Correction request:
- reinstall wattle along north side of garage on State Street to be staked every 4-5' and be plush to ground with 2" trench.

- work with St. Luke's to prevent compactor leak from their facility from comingling with stormwater from the construction site.

- site is using Ecopan service for concrete washout
- trash is well managed, no loose trash laying around site
- site has sweeping unit working at access points. Active haul off occuring at Jefferson




Corrections requests:
- install gravel bag gutter check dam bags on Jefferson where CUP excavation & haul off is occuring to contain material in gutters close to site for easy manual clean-up
- check with St. Luke's facilty management regarding putting spill absorbent tubes or secondary containment around the trash compactors (due to leaks and oil drips)
- check that SWPPP has completed operator certfication paperwork complete.
- Inspection reports should be placed in SWPPP binder no longer than after 14 days of the date of inspection
- request during site training or employee meetings that offsite smokers do not throw cigarette butts on the ground but into a trash container or ash tray

- Install stabilized access to the Ecopan’s located on 1st street on east side of garage structure, scoop/contain mud along the curb

No trackout, discharges, or other violations observed. SWPPP onsite and site inspections are every 2 weeks and after rain events.

Site walk around and inspection with SWPPP coordinator Charlie of the 3 St. Luke's project. Site stormwater is retained on site, existing DIs are disconnected and or flow into the excavation of the site. Ecopan concrete washouts are utilizied and in good condition.

Jefferson Street to the north of project will soon be demolished/excavated. Streets directs stormwater into site.

Corrections:
- install gravel/road mix on loading areas and temporary parking spots around perimeter (bannock & 2nd)
- re-grade corner access and stabilize
- need to reinstall wattle along the stockpile/backfill piled up along Bannock. Wattle should be installed at toe of slope, be trenched in 2-3" and staked every 4-5 ft.
- make sure inspection records are up to date. An inspection form should be in binder within 1 week of the inspection date.
