Will install large lined berm at bottom of road to direct all runoff into the retention pond. Pond overflow to be capped and pond level monitored.

Additional lot soil roughening and berms to be added on lots along Eyrie. Upper area should remain undisturbed other than some stormwater volume and velocity controls

RP had wattles installed around the disturbed area from recent utility work and will be hydroseeded.

Lots above Arrow Villa should not hold water close to the ridge above existing homes, but should allow water to flow towards the unfinished road for storage and infiltration there.

No violations observed, RP requested curbing crew to demand concrete truck drivers use lined washout facility for all washout activiities.

WIP on joint trench. Talked to RP and she thinks they should be able to pave this fall.
Work on section of road leading to phase 8 where the bollards use to be. Widening and adding sidewalks per ACHD, also some repair at bottom.

No new activity. No runoff from site, all water obsorbed or directed into pond.

No activity. No blowing dust, trackout, or issues. Owner wants to pave site before winter.

Small crew at top of site. Lower section is not active. Damaged areas from draining pool have been fixed. No trackout or dust, few stockpiles on site. Hillside vegetation doing good, few spots will need repairs.

Site is clean and contained. Good entrances and top and bottom. WIP on sewer, hopes to have all utilities, sidewalk, and paving done by October 15th.

Site is mobilizing to install utiilities, complete Eyrie Way, and widen and complete Arrow Villa extension to Winter Camp to finish phase 10.

ESC corrections in progress, discussed with site RP who will notify operators and developer:
- install/improve lower and upper construction entrances with 50 ft min length and heavy duty rock. Sweep loose dust at entrances regularly to prevent windblow fugitive dust.
- ensure that water truck is onsite today to control fugitve dust as necessary along haul roads and on material stockpiles.
- post new ESC sign on both upper and lower entrance.

Other comments:
- all construction traffic that can safely enter site from the lower entrance or Arrow Crest should do so to avoid using Arrow Villa. Employees should park at bottom entrance, project managers should travel through construction site.
- Arrow Villa extension should remain closed to the public and other thru traffic until Eyrie Way is open.

No water made it to pond, all absorbed by growing hillside vegetation. No major issues at site.

Site is absorbing water, very little making it to retention pond. Not much grass growing. Some small rills beginning to form. No major issues.

No runoff or failures. All water contained on site.

Met with Jill and Collin on site. Retention on bottom of road, not pond, overfilled. Some turbid discharge, goes to retention pond not creek. Washed gravel was ordered to make a berm and filter before road. Once dried berm on retention will be repaired and incresed in height.

No major issues, nothing leaving site. Pile of old silt fence laying on sidewalk at bottom entrance.

BMPs working. No sediment runoff or hillside erosion observed. No corrections at this time

Bollards have been ran over, curb broke, and some tracking.
Wire with survey tape has been strung across sledding hill and around upper area to try and keep people off. Hillsides are still stable, no runoff from bottom. Did not walk site due to mud.

Met with RP and did site walkthrough.
Wattle has been properly installed on slopes, trenched in and overlapped ends. One area towards bottom will need to be extended by 75', RP will take care of it.
VSI hydroseeding on site, starting on slopes with seed mulch and tackifier.

Ran into RP doing inspection while on site. She will come Saturday to do some BMP modifications while workers are off. No dust while working. Some water in pond at bottom, looks like tank is being drained with hose. No trackout from lower entrance.

WIP on grading and moving fill from ridge to lower areas of subdivsion phase. Water wagons circulating with belly scrapers, no fugitive dust clouds leaving site. NO trackout from site access points. A water diversion trench has been installed at botton of canyon and Eyrie Way to direct any runoff coming down into the stormwater/sediment retention pond.

Ridge work is being conducted per protocol, 2-3 foot shelf left to prevent and rockfall or clods from rolling down hill. Safety fence appears in good condition, area of slope disturbance from water truck misfire has had ECBs and slope interupting wattles installed.

Approved, site is in compliance.

Work just getting started for morning. Water wagon is leading way with dozer using ripper tooth behind it. There is still a high berm between grading area and hillside, all workers in low area away from edge.
Pond at lower section has been enlarged, water tank stored above, will drain into retention if it fails. Silt fence on bottom, corner post has been broken in half. No trackout.

Met with Adam, superintendent of site and did walk through. He has crew walking fence line and taking pictures for time stamp proof of inspection.
Excavators are cutting down inside of hill and leaving a bench. Site foreman is running the smaller D6 dozer in mornings to level out cut. Dozer is running parallel to hillside with spotter. Dozer being used has ability to angle blade and is pushing everything away from homes and into downhill side of ravine. From bottom of hill may be difficult to see what work is actually being done and may look like a flat cut not being angled. All of area is sloped toward ravine like discussed, no major rockfall towards barrier fence seen.

Report of landslide received. Incident reported via email on 9/15 PM.

Inspection conducted 9:15 AM Monday 9/17.

Earth work subcontractor L3 had a water truck working on Saturday morning 9/15 to soak water in on the area of planned excavation on Monday. Plan was to get ahead of fugitive dust and make the material more easy to grade/doze. Water truck driver did not observe equipment limits toward the edge of rim at got stuck in the mud while spraying water inadvertantly over the berm and down the slope towards Arrow Villa. Force of the water spray caused uprooted grass and mud to flow down a previous compacted area (equipement tracks from 2-3 years ago). The rockfall safety fence stopped a majority of mud and debris. Some water and mud did flow under the fence down to the toe of the slope depositing sediment in the pond. Does not appear any water flowed further into pond towards pond outlet.

Corrections:
- remove captured mud and debris and fix fence. Re-secure fence posts and staples on either side of where the mud is located. Rake out compacted mud channel.
- remove any stockpiles or pushed up dirt from edge of rim
- install temporary sediment control and slope interupting wattles on the eroded area. In 1-2 weeks the slope shall be tracked and hydroseeded. Requested that permission be obtained through ACHD.
- remove deposited sediment in the edge of pond area at bottom of slope (approximately 1/3 of a cubic yard)

- no further advance soaking into the ridge is allowed. Water trucks and other equipment are restricted from working or driving directly upon the edge. L3 is to delineate this restricted area. Water truck operator is no longer to work unless on L3 field supervisor is on duty.

- each day a safety meeting and onsite supervisor (Taylor or Adam with L3) shall walk the area of work and fence sections to inspect potential issues. Additionally the rim should be inspected at the end of each activie work day to make sure there is no ponding, low spots, or loose rock or clods that could potentially roll down hill.

- All grades (including temporary) on the rim shall be done in a way that directs & drains water away from the slope on the south side of the rim and towards the north to the valley area of phase 10.

Grading has begun above the lower homes and down the canyon. Lots of dust on top, falling into draw away from homes, water wagon is spraying site. Still a large buffer and no grading on the downhill slope torwards homes.
No issues at lower entrance.
