Silt fence has been removed, wood post and a t-post still installed.
Heard from source that drainage is flowing wrong way and onto hillside.

No runoff or erosion issues observed.

Request to remove silt fence below fence has not been completed.

Driveway repaired. Still some slurry on hillside.

Driveway has been repaired. Mess on hillside below has been cleaned up. Hillside has some rills from blowout and will need some repairs.

Storm drain sumps in road way and for the house drainage system have been hydro-vac'd.

Need to get geotechnical report on slope that was damaged during the water line break in early July.

Driveway has been cut out and being evaluated by soils engineer. Some saw cutting slurry has gone down drain and bubbled up below, some also has gone out drain into hillside. Talked with RP and will see what ACHD wants done and if they need to vac out drains. Broken concrete is currently in road.

Washout has been cleaned and new washout installed.
Gutters on home have been hooked up into system. No rills or runoff from site. Lower drainage is working, will be filled with drainage rock for appearances.

Follow up from failed inspection

Notified Cleve with Roth that adjacent site where washout from driveway placement was dumped must be cleaned up and a new washout installed.

Entry error

Inspected 3/26. Spoke with Roth Homes foreman. All material off of road, said they had some mud but cleaned it all.

No workers on site, no materials on road. Did not walk hillside due to rain and mud.

Pile of trash out front hasn't been removed.

Some corrections have been completed:

- Drainage swale has been cleaned out and re-lined as requested.
- Trash on hillside by home has been picked up
- wattles on the west side of home have been replaced and new ones installed.
- wattle placed below swale as requested (needs some re-installation to trenched and along contour)

Corrections remaining:
- clean up trash pile in front of home
- fix wattles below swale
- install storm drain grate in correct direction

Follow up meeting to be scheduled with Kevin from Roth Homes.

Washout in front was never cleaned. Drain below home looks full of sand. Hillside work on top pushed some fill over wattles and disturbed vegetation.

Some correction completed:

- stockpile moved out of gutter
- upper DI bag emptied and re-installed
- lined swale has been cleaned out with dirt and trash removed as requested.


Not completed:
- spill absorbent and planks obstructing lower drains at end of culdesac.

Load of pitrun has been dumped halfway on road, on top of protected storm drain. Sand bag around lower drain is starting to fail and lose sand, inlet has witches hat. Hillside is stable.

Hillsides are stable. New trenches for power, stockpile on site, very close to being in road. Unable to check drainage below home, trench blocked access on one side, scaffolding on other. Trash is contained at both sites, right of way not being used for storage. No ESC issues at either site.

No sediment runoff observed or reported. Slopes and drainage system have not been overwhelmed by recent storms.

Correction request:
- clean up accumulated dirt in lined swale below home
- remove unused wattles around site
- continue to work with neighbor on tree replacement

WIP on interior framing.

No new violations observed. Disturbed slopes are in process of revegetation. No significant bare areas or erosion rills observed. Final landscaping has not been completed. Foundation and frame complete, WIP on interior.
None of the corrections requested in March appear have been completed other than making the lined drain flow towards the outlet pipe when water collects in it.

Corrections needed:
- remove unused wattle from bottom corner of lot
- install properly trenched in wattle between the drain and bottom corner.
- work with neighbor to replace damaged tree and gravel in driveway as discussed.

Site contained, hillside grasses coming in. No ESC issues.

Lined drainage trench has been cleaned out and relaid to improve flow to drain and prevent overtopping. Front of site is clean, no trackout and trash container has been emptied.

Corrections requested:
- landscaper foreman/crew to discuss tree replacement and revegetation with downhill neighbor
- install wattle U-hooks/check dams along bottom fence line to slow any water flowing along the fence liner.
- clean up old/unused wattles at discharge point at corner of fence.
- install section of wattle on slope between the drainage trench and bottom corner of lot.

Skylark street is in good condtion, no trackout, trash contained, storm drains have sediment protection in designated inlets.



Disturbed slopes have jute matting and seeding in place with slope interupting wattles. Wattles are not overlapping , will continue to monitor tp see of water is channeling there.

Drainage swale may need re-installation, water is ponding in low spot in trench instead of going drain.

Damage on lot below home from previous runoff events discussed with neighbor.
- dead tree and damaged vegetation in area where mud ran down hill. Homeowner wants tree replacement and reseeding.
- mud/dirt deposited in driveway and between garage and slope has changed grade and is creating ponding and mud. Regrading and import gravel requested.

RP has been notified to reach out to neighbor and check into swale/drainage channel below home.

Hillside BMPs still in place. No runoff onto road. Trash and lumber is being put into trash box. Some lumber is being stored on road in cul-de-sac.

Check for clean right of way.
Everything has been cleaned off road.

Hillside BMPs are holding up, no issues with hills. Sonme dirt on road and in gutters. Is coming from home above new properties.

Trackout cleaned. Hillside now has jute, wattle, and has been hydro seeded.

Excessive trackout on road. No protection on steep slopes.

No dirt leaving site. Hillsides are expose and unprotected.

No permit displayed or ESCP/SWPPP book. Entire hillside is bare and loose, no BMPs to prevent water from racing downhill. Would install swales to divert water or wattle and blanket to protect dirt. Would like to see tackifier or hydro seed before winter weather. One of four DIs is not protected. Is next to 140 Skylark which shares responsibility for hillside.

Concrete slurry has been cleaned up and trackout swept. Trash is still full.

WIP on road, curb, and gutters. Staging lane is clean, trash is contained. No ESC corrections observed.
Concrete slurry flowing down side of hill to vegetation. Silt fence ok. No curb installed under fence. Trackout. Wattles not staked. Overflowing trash. No dust problems at time of inspection.

Clean up concrete washout that has spilled downhill. Install concrete washout to properly contain concrete. Communicate use to subcontractors

Requested corrections have been completed:
- trash container removed.
- street sweeping on Skylark
- silt fence section has been repaired. Still needs backfill to trench in
- upper stockpile has perimeter protection and has been removed off of curbing and adjacent driveway
- excavated mud removed from neighbors has been cleared.



Site is now back in ESC compliance.
Further adjustments:
- backfill silt fence section
- install J-hooks on wattles along
- stake down all wattles strips not currently staked

Fence is still not installed. No other issues.

No new developments. Fence still not finished.

Wattles have been replaced on roadway. Silt fence still has sediment on top of it. Nothing else new.

A portion of the silt fence at the bottom of the site has been removed. Left message with RP requesting maintenance on silt fence.

Silt fence in good condition. No run off to lot downhill of site. Wattles are placed on roadway to direct and slow water. Addition gravel of construction entrance is on site. No trackout. Sign is posted. No work currently underway, but an excavator is on site. No corrections requested

First phase of construction is to install permanent heavy duty fencing and stormwater swale at bottom of lot for drainage. Perimeter silt fence has been installed properly per ESC plan.

No DI protection to be installed due to slope of road and catchment area for storm drain. Do not want to overwhelm or clog inlets which could send stormwater over the hillside or onto existing property. RP Cleve to increase gutter and street sweeping frequency.

Approved for construction.
