Observed on 1/10 in PM runoff was not reaching the subdivision inlets and blocked by snow and ice. Runoff was running down street beyond subdivsion stormwater facilities. Runoff was clear.

RP fulfilled correction request by having crew clear gutters and flow into inlets in the subdivision to capture and treat runoff. Crew chief stated inlet bags were also removed and new bags installed.

- after paving and re-distribution of stockpiles (to be done this week and next), slope interuption/sediment control wattles will be installed on hillsides. For erosion control, a "tacifier with minimal mulch" will be used for stabilization until Sept-Oct when hydroseeding application

Gutters and storm drain inlets were cleaned out and sediment filters re-installed as requested.

Additional sediment control wattles and check dams were installed at bottom of site to filter runoff.

Graded area that was added to permit has had slope stabilization and sediment control wattles installed. RP stated that disturbed slopes no longer with work remaining are to be hydroseeded this April.

SWO lifted, remaining corrections include:
- pay re-inspection fee
- install new temporary retention pond somewhere in middle of the project to capture and slow down runoff from the upper end of project so that there is not some much runoff exitting site at bottom all at once.

Pat stated he will make these corrections asap.

Approved for construction to resume. GRD and ESC permit now in compliance with submitted plans.

Complaint called in to Public Works Hillside regarding polluted runoff from site.

Runoff was no longer flowing at time of inspection but there is evidence of a significant muddy flow and collected mud at 5th and Ada in particular:

Reason for BMP failure is that no new BMPs were installed to replace the sediment retention pond and wattles that were installed prior to the sewer work being completed. Runoff moved down the compacted road base picking up sediment and flowing onto Sherman street untreated.

Correction notice issued:
- clean up streets and intersection from site to 7th & Ada MS4 DI.
- install BMPs at bottom of site to control sheet flow runoff from higher areas.
- street sweeper shall be required.

Note: site has SWO on it due to grading re-submittal request. ESC plan currently under review. No workers or activity at site.

Inspection conducted to check conditions for potential snow melt runoff. No discharges from site, snow on upper portions not yet melted. Retention pond at bottom of site was nearly empty showing a decent infiltration rate and adding capacity for snow melt.

Work on placing manholes has been occuring this week by Knife River. Some trackout to Sherman and the access to the private driveway has occured. Onsite KR supervisor Tim had a crew starting to clean it up as best as possible in the wet & snowy conditions. Also resquested that KR make a diversion channel from the finished section of road to make sure flows are directed to the retention pond and wattles instead of the street.

Approved for now. Will check back for progress and conditions. Very sensitive site.

Checked BMP performance and site conditions. Retentions ponds were able to retain runoff from upper portions of site. No muddy blowouts but fortunate there was not much more rain. Although installed BMPs and corrections implemented Tuesday improved sediment control at site, significant amount of prohibited discharge running off site and down Sherman Street to Ada.

Upon arrival at approximately 9:30 AM Syman Crew had just left site stating that wattles were working well. Inspection of wattles and site found many failures where water was undercutting wattles. Wattles are definitely lengthening the time of concentration of runoff at the discharge point, but is not effectively trapping and removing sediment.

The problem is too much water is coming down the future road area for the wattles handle effectively. Water is from upper regions is bypassing the first retention pond and quickly filling up the second and final pond. Additionally some of the runoff was by-passing the last pond right along the edge of pavement and overwhelming the last section of wattles and sandbags.

Notified RP of need to improve retention pond setup. Knife River contacted me to let me know a new pond can be installed tomorrow as needed.

Worked at site for approximately 2 hours fixing some issues and diverting water flows. ACHD inspector and I stopped the flow of water bypassing the last retention pond and dug a channel to direct water into the pond.

During this time of active discharge the RP and no other subcontractors arrived to assist with trying to stop run off, which was surprising after the large storm we had received and the issues earlier in the week.

Corrections:
- further work needs to be done to make sure we have adequate retention of water and that runoff is being directed equally to both ponds.
- need to clean up and sweep around the muddy tractor on Sherman.
- need to address the area to the right of the main entrance at end of private driveway area
- when bottom pond drains, it needs to be lined with geotextile fabric and the inlet & outlet reinforced with rip rap.
- not clear if any sweeping was conducted on 12/9 as requested. Need documentation/invoice that this was completed.

- Due to time spent making makeshift corrections a Special Investigation Fee is being charged to permit.

9:00 AM Site walk-through inspection of BMPs installed for corrective action in response to prohibited discharge on Monday 12/7 during significant rain event

Site was shut down on Monday afternoon and inspected by EPA for Clean Water Act enforcement inspection on Tuesday 12/8.

Reinspection fee paid and RP name changed to Pat Boel.

Corrective actions have been made and documented by site stormwater team. Corrections competed:
- straw wattles have been trenched, staked down and placed at 25-30 foot intervals on all graded areas of site to capture sediment, reduce slope length and slow down flow off runoff from upper sections of site to allow for infiltration and diversion. A wattle was also installed to treat runoff from graded area above the road footprint.

Two sediment traps/retention & infiltration pits have been installed near the bottom of the site. The upper pit was not lined at time of inspection, the spillway has rip rap and the discharge from pond then passes through more wattle and into a lined pit with basalt rock. The site hopes that all runoff will be contained onsite instead of discharge. There is a lined outlet in case of overflow.
-Syman Co. will be onsite to observe rain fall and sheet flow during the next event. They will conduct maintenance as needed and extra wattles and materials are on site.
- MS4 for inlets below site have had sediment filter bags installed.
- sand bag check dams have been place on the drainage route on the private driveway extension of Sherman to the right of site. Driveway are has been cleaned of mud and some road mix sand/gravel has been installed. Additional work and installation of berm or wattles has been discussed with Syman Co. Andy Hoak

Corrections still in progress:

-Street sweeping along Sherman and Ada is required before end of day 12/9. Additional inlets and mud collected in front of homes and intersections should be cleaned as much as possible.
- Gravel check dams need to be installed uphill from the upper holding pit along the base of the slope. Water was channeling and flowing fast in this area on Monday

Will conduct inspection during next significant rainfall to observe BMP performance.

Stop Work Order removed, involved parties notified. Approved for manhole and sewer line installation.

Prohibited discharge incident is still under review and pending potential further action.
