RP reported broken water line over weekend.

Posted Corrrection Notice requests on 200 and 220 W. Sherman to sweep up loose dirt in roadway.

No permits filed, investigation pending. No contact with homeowners at this time.

No recent activity since prior inspection. BMPs in good condition. No erosion or runoff issues observed ( no rain received either). SFHs not yet in progress.

No recent activity or construction. All subdivison development work is complete and BMPs are installed and in good condition. BMPs include DI bags, wattles, and a temporary tacifier application.

SFH should begin this Fall, RP with Local Construct stated that in the fall, all lots and open areas not under construction will be hydroseeded in Oct/Nov. Will keep this permit open until this has occured. No corrections, no issues observed.

Corrections are needed:
- pick up all unused BMPs (wattles) laying around site
- complete sweeping up dirt from site collected in gutter on Sherman and at the divergence on Sherman and Ada

- pick up uncontained trash pile towards top of project and any other loose trash around site

- install sediment control BMP or diversion to prevent muddy water from the uncompleted portion of the street to flow down through old portion of Sherman and into Northend.

- conduct site walk through conference to discuss plan to stabilize slopes to prevent erosion and sediment runoff (the proposed meeting)

Initial complaint/notification came from Tony Young, then EPA about an Illicit Discharge coming from site.

Discharge came from Fire Hydrant flushing with no sediment filtration in place over loose sediment. Knife River are beginning to pave streets and pour curb, gutter and sidewalk. Drop inlet protection in place at bottom of sherman st. No more discharge coming off of site.

Dust Complaint from Trucks Hauling out soil

Significant amount of dust being kicked up from mass hauling. Talked with RP to get the water truck spaying down the fine sediment. He jumped right in the water truck and began to water down the road and area.

Check up on site after heavy rain storms that occured throughout valley last night.

No turbid discharges or significant sediment loads carried from site evident. Wattles and check dams installed last week have captured/treated some runoff. No additional retention pond was installed as recommended/discussed, but it appears BMPs and perhaps a less intense rainfall in this area have left the site in compliance.

Complaint called in to Public Works Hillside regarding polluted runoff from site.

Runoff was no longer flowing at time of inspection but there is evidence of a significant muddy flow and collected mud at 5th and Ada in particular:

Reason for BMP failure is that no new BMPs were installed to replace the sediment retention pond and wattles that were installed prior to the sewer work being completed. Runoff moved down the compacted road base picking up sediment and flowing onto Sherman street untreated.

Correction notice issued:
- clean up streets and intersection from site to 7th & Ada MS4 DI.
- install BMPs at bottom of site to control sheet flow runoff from higher areas.
- street sweeper shall be required.

Note: site has SWO on it due to grading re-submittal request. ESC plan currently under review. No workers or activity at site.

Syman Co. to respond to clean up issues this afternoon.

Notifiy RP-Local Construct that there are outstanding frees

Contacted Patrick via phone and left VM that there are outstanding fees that need to be paid asap. No issues observed at this time

No discharges leaving the site. BMP's installed. Culdesac work is occuring at this time. No ESC issues observed at this time

Inspection conducted to check conditions for potential snow melt runoff. No discharges from site, snow on upper portions not yet melted. Retention pond at bottom of site was nearly empty showing a decent infiltration rate and adding capacity for snow melt.

Work on placing manholes has been occuring this week by Knife River. Some trackout to Sherman and the access to the private driveway has occured. Onsite KR supervisor Tim had a crew starting to clean it up as best as possible in the wet & snowy conditions. Also resquested that KR make a diversion channel from the finished section of road to make sure flows are directed to the retention pond and wattles instead of the street.

Approved for now. Will check back for progress and conditions. Very sensitive site.

No discharge from discharge point. New BMPs effective but pond is still not sustainable in the event of more prolonged rain storms.

ESC crew onsite doing maintenance on wattles failures, removing vehicle ruts on road that are channelling and concentrating water, and digging diversion trench to get water into the upper pond.

Approved for now, will continue to monitor.

9:00 AM Site walk-through inspection of BMPs installed for corrective action in response to prohibited discharge on Monday 12/7 during significant rain event

Site was shut down on Monday afternoon and inspected by EPA for Clean Water Act enforcement inspection on Tuesday 12/8.

Reinspection fee paid and RP name changed to Pat Boel.

Corrective actions have been made and documented by site stormwater team. Corrections competed:
- straw wattles have been trenched, staked down and placed at 25-30 foot intervals on all graded areas of site to capture sediment, reduce slope length and slow down flow off runoff from upper sections of site to allow for infiltration and diversion. A wattle was also installed to treat runoff from graded area above the road footprint.

Two sediment traps/retention & infiltration pits have been installed near the bottom of the site. The upper pit was not lined at time of inspection, the spillway has rip rap and the discharge from pond then passes through more wattle and into a lined pit with basalt rock. The site hopes that all runoff will be contained onsite instead of discharge. There is a lined outlet in case of overflow.
-Syman Co. will be onsite to observe rain fall and sheet flow during the next event. They will conduct maintenance as needed and extra wattles and materials are on site.
- MS4 for inlets below site have had sediment filter bags installed.
- sand bag check dams have been place on the drainage route on the private driveway extension of Sherman to the right of site. Driveway are has been cleaned of mud and some road mix sand/gravel has been installed. Additional work and installation of berm or wattles has been discussed with Syman Co. Andy Hoak

Corrections still in progress:

-Street sweeping along Sherman and Ada is required before end of day 12/9. Additional inlets and mud collected in front of homes and intersections should be cleaned as much as possible.
- Gravel check dams need to be installed uphill from the upper holding pit along the base of the slope. Water was channeling and flowing fast in this area on Monday

Will conduct inspection during next significant rainfall to observe BMP performance.

Stop Work Order removed, involved parties notified. Approved for manhole and sewer line installation.

Prohibited discharge incident is still under review and pending potential further action.

Continued BMP failure and sediment runoff down Sheran and discharging into inlet on Ada just past 7th Street.
